There are a variety of documents that could come before a BA is engaged, although often senior BAs are a part of creating those documents. A business case is likely the most generic term and might have a business capabilities analysis, SWOT analysis, financial analysis, etc. In Scaled Agile, it might be an epic hypothesis statement. This is a template for what YOU should capture on YOUR project. The kinds of questions you should ask, the kinds of things you should be thinking about. Most companies who require a BRD will have a very similar template for you to follow and I have used this structure before a few times. How well it works in the real world is dependent on your ability to think through and capture information fully and accurately
In agile you wouldn't attempt to get full detail on every single requirement. You have a high-level direction and as the product gets built requirements get added or deprioritized as you learn more and the environment changes.
If by MoM you mean minutes of the meeting, then no. Every meeting can be pretty different in nature, so I take notes based on what I need to understand or what others will need to understand later.
