Had to do that foam slab repair at work. The void under the slab was so large that cut an access hole and back filled with sand. A guy was inside with a wheel barrel, that how big. So it could have been worse.
Great vid! - So did the injected foam actually lift the floor in the low points or just fill in and keep it from dropping further? If the former, how did they control how much it raised it?
One thing that works better than threaded rods and epoxy is the Titen HDs. They are basically HUGE tapcons for concrete. Easy in, Easy out, SUPER hold. I defy you to find something it cant hold.
