I have been a dentist for decades now, and of course the trigeminal nerve is paramount to the practice of dentistry, However my understanding was superficial. I went back for my Master's in Pain management and began looking for videos that could explain the trigeminal in detail--there are a bunch of superficial ones out there. There are only a couple with the detail, excellent anatomic illustrations, and clear explanation needed by someone who needs advanced knowledge. This is one of them. It is great for everyone else too--but it remains one of my go to videos for neuanatomic detail. Kudos for such a great vid!

I am studying to take my INBDE exam and the cranial nerves are a key part of this exam, I must know, understand and clinically relate all the aspects of these cranial nerves, specifically the trigeminal and facial, and this class is the best that I could have happen. Small correction, but at 17:56 the more medial branch of the frontal nerve is the Supratrochlear, whereas the more lateral branch is the Supraorbital. Just saying because I missed a tagged structure because I got the two mixed up. But all in all this was a phenomenal video to learn about the trigeminal nerve for my upcoming exam, and definitely beat the lecture explanation of this! Thank you, you explained everything in a way i could understand, I have suffered from trigeminal neuralgia for 5 years and I never understood how it worked, according to this mine is the lingual nerve as it affects my tongue so any chewing/talking/swallowing would trigger an attack.

I've been dx with tic douloureux. (Comorbidity of ms). Funny enough, I've stumbled upon your video while browsing your amazing work, and after 4 years, it's the absolute very first time that I'm getting comprehensive education about the anatomy of it all. Let me testify that it's a powerful mf .. For those who never heard of tic douloureux, it's a chronic neuralgia. Also called the suicide disease 🙃
Just going through a flare-up myself- oddly I think I diagnosed myself jut I did get confirmation from my primary care physician but I'm scheduled with my neurologist also. She's 50 miles away so a little harder to get scheduled. And yes- it can hurt so bad you want to decapitate yourself! PCP somewhat joked That would be a very permanent solution to a temporary condition! Praying it IS ONLY Temporary right now+
You're knowledge of the trigeminal nerves are amazing! I'm watching this because my wife has burning mouth syndrome. Her palate, tongue, and teeth are burning continuously at extreme pain levels. Medications are no longer managing the pain. We are working with neurologists, GP, etc. all smart people that have run out of options for my wife. Because her pain is through maxillary and mandibular nerves this seems to cause confusion on where to look for treatment. What is your thought on her nerve pain?
Really good to understand the physiology of the trigeminal nerve. You mention Trigeminal Neuralgia and how awful it is, and its treatment. However there's something worse than TN. It's called Trigeminal Neuropathic Pain. This is nothing to do with the interaction with the blood vessel but is caused by damage along the nerve branches. As opposed to TN where you can have short sharp stabbing pain up to 100 times per day, TNP sufferers are in continual pain. Agony. Just the most horrific pain, unceasing. It is often mistaken for TN and treated incorrectly with the patient getting little relief from Gabapentin or Carbamazepin. The best treatment is to inject 100mg depomedrone along the course of the nerve which will reduce pain levels for a few months before needing to be repeated. Some doctors use Botox as an alternative to steroids, but personally I find it is far less effective. TNP is often missed by physicians so it is vital that you check if the pain is intermittent or continuous. I watched this because in the last few weeks my TNP has spread to my ear and I wanted to check if it was my trigeminal nerve being more of a dick, which you seem to have confirmed that it is. As a TNP sufferer myself, I ask you to remember the worst toothache you've ever had, the worst sinusitis you've ever had, the worst headache and the worst ear infection you've ever had. Put them all together, happening at the same time. Now imagine that pain forever, for the rest of your life. The nerve is damaged. It can't be fixed. It is telling you every second of every day that you are in pain. You can't have anyone or anything touching your face ever again. You dread getting a cold, or even just crying. You hate a windy day or just Chicago in general. Do not assume TN. Do consider antidepressants for the poor soul suffering because we know that death is the only true escape.
Oh you poor thing you .I feel horrible ear pain as if it’s middle ear infection ( I had that a few years prior to getting this facial pain) I had a car accident and my jaw was dislocated the pain became so bad I wanted to step out of life . Like you say touch , wind everything causes pain . My dentist saw something wrong and sent me to a physiotherapist. I have been there for 20 years . I had pain on the left side , I was involved in yet another car accident ( both not my fault these accidents) and the other side started the same pain . I have horrible ear pain in the ear like a huge pin gong into it according to my therapist this is also a nerve that causes this pain. I cover up my face in the winter keep my head warm . I also get cluster headaches besides all the other horrible pain. Some explain it as sort stabbing pain, I call it hell . My therapist said it is called suicide pain . I hope you have found something to help you with your pain . Much luck with your pain. Xxx
@@margaretpeelen3968 It's truly horrific, isn't it? I am sorry you suffer also. Over here in UK, TN is described as "the suicide disease" but there is nothing on TNP even on the NHS website. My consultant is one of only a few to have studied and found a halfway house treatment, but it took 10 years and countless referrals (even one to the best TN team in the country) before he just removed the veil and properly identified the illness and all my meds got changed, I got the injections he was trialling as a study and I finally thought there was a chance of not killing myself sooner rather than later. With all that, the pain is still there, but it's manageable for a while, and that's the best I can ask for. Unfortunately, it is the UK; the NHS which we are so proud of is critically under-resourced, Brexit caused us to lose thousands of European staff we relied so much upon and then covid meant treatments were delayed. The injections I should get every 3-4 months have a 2-year waiting list. I'm 9-months in and the pain is devastating. I know, however, that I would never have made it even to my first injections if I lived in America where you can buy a gun in Walmart. I would have blown my face off countless times. That said, I'd probably be getting my injections regularly with employer health insurance...

First Maxillary Molar is actually innervated by both Posterior Superior Alveolar Nerve ( Palatine and Disto buccal roots) and Middle Superior Alveolar Nerve ( Mesio Buccal root). Thank you for your lecture! The best! :)
Hi, I was looking through the comments to see if anyone had pointed that out. I partially agree, the first molar is innervated by 3 branches, the mesial root by MSA, the distal by PSA and the palatine by the palatine nerve. At least that's how they taught us, so for the anesthesia to take effect for the first upper molar, we have to aim for these three branches. Having 3 days left till my exam. Telling myself that one day for cranial nerves is enough. Coming to 5th nerve, feeling like im gonna need 3 days only for this one. if i fail the exam at least i ll know from where i ll be preparing in January.

I have all three branches that are damaged. Burning that goes all up over my head, also I have pin point pain spots. My eye is on fire, my fave is on fire. The pain is horrific. I am now on so much medication it is hard for my to deal with life. I am so happy you did these video's!

Small correction, but at 17:56 the more medial branch of the frontal nerve is the Supratrochlear, whereas the more lateral branch is the Supraorbital. Just saying because I missed a tagged structure because I got the two mixed up. But all in all this was a phenomenal video to learn about the trigeminal nerve for my upcoming exam, and definitely beat the lecture explanation of this! Thank you, you explained everything in a way i could understand, I have suffered from trigeminal neuralgia for 5 years and I never understood how it worked, according to this mine is the lingual nerve as it affects my tongue so any chewing/talking/swallowing would trigger an attack.

My wife age 30 suddenly 4 month befr, she lost vision in her right hand eye. When we went hospital and taking reports, doctor tell this is stocks affect's eye. Blood from brain to eye is block, then they remove the block. So now Nerves from brain to right hand eye is some damage. Nw my wife vision in left hand eye only. Only half vision in right hand eye. For those who never heard of tic douloureux, it's a chronic neuralgia. Also called the suicide disease 🙃
Just going through a flare-up myself- oddly I think I diagnosed myself jut I did get confirmation from my primary care physician but I'm scheduled with my neurologist also. She's 50 miles away so a little harder to get scheduled. And yes- it can hurt so bad you want to decapitate yourself! PCP somewhat joked

You're knowledge of the trigeminal nerves are amazing! I'm watching this because my wife has burning mouth syndrome. Her palate, tongue, and teeth are burning continuously at extreme pain levels. Medications are no longer managing the pain. We are working with neurologists, GP, etc. all smart people that have run out of options for my wife. Because her pain is through maxillary and mandibular nerves this seems to cause confusion on where to look for treatment. What is your thought on her nerve pain?
Really good to understand the physiology of the trigeminal nerve. You mention Trigeminal Neuralgia and how awful it is, and its treatment. However there's something worse than TN. It's called Trigeminal Neuropathic Pain. This is nothing to do with the interaction with the blood vessel but is caused by damage along the nerve branches. As opposed to TN where you can have short sharp stabbing pain up to 100 times per day, TNP sufferers are in continual pain. Agony. Just the most horrific pain, unceasing. It is often mistaken for TN and treated incorrectly with the patient getting little relief from Gabapentin or Carbamazepin. The best treatment is to inject 100mg depomedrone along the course of the nerve which will reduce pain levels for a few months before needing to be repeated. Some doctors use Botox as an alternative to steroids, but personally I find it is far less effective. TNP is often missed by physicians so it is vital that you check if the pain is intermittent or continuous.
I watched this because in the last few weeks my TNP has spread to my ear and I wanted to check if it was my trigeminal nerve being more of a dick, which you seem to have confirmed that it is. As a TNP sufferer myself, I ask you to remember the worst toothache you've ever had, the worst sinusitis you've ever had, the worst headache and the worst ear infection you've ever had. Put them all together, happening at the same time. Now imagine that pain forever, for the rest of your life. The nerve is damaged. It can't be fixed. It is telling you every second of every day that you are in pain. You can't have anyone or anything touching your face ever again. You dread getting a cold, or even just crying. You hate a windy day or just Chicago in general. Do not assume TN. Do consider antidepressants for the poor soul suffering because we know that death is the only true escape.
Oh you poor thing you .I feel horrible ear pain as if it’s middle ear infection ( I had that a few years prior to getting this facial pain) I had a car accident and my jaw was dislocated the pain became so bad I wanted to step out of life . Like you say touch , wind everything causes pain . My dentist saw something wrong and sent me to a physiotherapist. I have been there for 20 years . I had pain on the left side , I was involved in yet another car accident ( both not my fault these accidents) and the other side started the same pain . I have horrible ear pain in the ear like a huge pin gong into it according to my therapist this is also a nerve that causes this pain. I cover up my face in the winter keep my head warm . I also get cluster headaches besides all the other horrible pain. Some explain it as sort stabbing pain, I call it hell . My therapist said it is called suicide pain . I hope you have found something to help you with your pain . Much luck with your pain. Xxx
@@margaretpeelen3968 It's truly horrific, isn't it? I am sorry you suffer also. Over here in UK, TN is described as "the suicide disease" but there is nothing on TNP even on the NHS website. My consultant is one of only a few to have studied and found a halfway house treatment, but it took 10 years and countless referrals (even one to the best TN team in the country) before he just removed the veil and properly identified the illness and all my meds got changed, I got the injections he was trialling as a study and I finally thought there was a chance of not killing myself sooner rather than later. With all that, the pain is still there, but it's manageable for a while, and that's the best I can ask for. Unfortunately, it is the UK; the NHS which we are so proud of is critically under-resourced, Brexit caused us to lose thousands of European staff we relied so much upon and then covid meant treatments were delayed. The injections I should get every 3-4 months have a 2-year waiting list. I'm 9-months in and the pain is devastating. I know, however, that I would never have made it even to my first injections if I lived in America where you can buy a gun in Walmart. I would have blown my face off countless times. That said, I'd probably be getting my injections regularly with employer health insurance...

First Maxillary Molar is actually innervated by both Posterior Superior Alveolar Nerve ( Palatine and Disto buccal roots) and Middle Superior Alveolar Nerve ( Mesio Buccal root). Thank you for your lecture! The best! :)
Hi, I was looking through the comments to see if anyone had pointed that out. I partially agree, the first molar is innervated by 3 branches, the mesial root by MSA, the distal by PSA and the palatine by the palatine nerve. At least that's how they taught us, so for the anesthesia to take effect for the first upper molar, we have to aim for these three branches.

@@user-xx7ed7ti1z Actually Palatine root is also innervated by Posterior Superior Alveolar Nerve. The reason you have to anesthetize Palatine nerve is because of palatine mucosa. Only for extraction. For restoration just PSA and MSA you'll be fine. Thank you for your comment :)
