the grammar is correct. i´m german and i don´t hear grammar mistakes. German grammar is quite a bit different than English. in the last line is some kind of stilistic device "the memory you gave me is still here" (completely literally) correct German grammar would be "hier ist das Vergissmeinicht, das du mir gegeben hast" (here is the memory you gave me) but it´s correct too. it´s a pretty typical device to chance the parts of a sentence like that
