    Can't thank you enough for your detailed comprehensive explanations. I stroked a little over 6 mos. ago. Weakness left side, but walk, talk, & chew gum at the same time, however, I notice I waddle (compensatory?) unless I put that Posterior Kinetic Chain to work. Now I know why. And this is a video I need to re-watch a time or two to get my new neurons working around some of the concepts you are giving us. But from the bottom of my heart Thank You for what you do. I felt so abandoned 3 mos ago. If it hadn't of been for you and your videos.... I'd hate to think of the result. You saved me. Thank You

    9:00 Kneeling band wrapped around shoulder, rotation away from weaker hip.
    20:15 one leg band pull while kneeling
    21:27 standing arm pull, one leg in front of the other

    Thanks for this, quite demanding, but well-explained video. I reviewed some parts again and tested to see if I understood correctly. I didn't know that they were called kinetic chains, but I felt them and called that connection for myself the force of action and reaction according to the physical principle. When I slowly relax my hand and walk due to spasticity, I would say naturally as you explained, it is much harder and more tiring than when I put the affected hand in my pocket or fix it with the other hand. I’m sure these exercises will help me get rid of stiffness and can walk relaxed naturally. Thanks a lot, I'll steal your cute puppy. Have a nice day.

      Hi Darko! Yes, this is one you might need to watch a couple of times to fully understand this. But once you understand this, I am super confident that you will come up with a whole bunch of movements to practice to improve your walking :)

    The biggest mistake I made during my recovery was not doing the bridge exercises sooner. Once I started doing bridges I noticed right away that my walking changed .

    This is wonderful and valuable information-thank you so much. My question is how to stabilize the weak leg in the kneeling exercises. My 84-yr old dad had a stroke and is working hard to get back his mobility. Would you point me to any videos on modifications for these exercises for those who haven’t regained enough strength to kneel…?

    I applied more closed kinetic chain exercises to my stroke patients for very long time and it's really helpful for them. I really like all your videos, you added some ideas to my practice. I'm looking forward to see videos about pusher syndrome and thalamic stroke. Thanks, you are one of the best neurologic physiotherapist. Keep up the good work.❣️

      Hi Leslie! I am glad I am able to give you some fresh ideas! I like to take old concepts and look at them with "fresh eyes". :) I plan to do one on pusher syndrome this month. Stay tuned! As far as thalamic stroke, where are your sticking points? In other words, what are the barriers that you have come across when progressing your patients? Pain, stiffness, a specific movement problem...etc.

    good explanation....exercises. Anatomy pics big help. i have Adult ADD and easily get bored so to make me do workout' more ' i like spending as much of my working' day as possible on the ground ..on carpet or other pad of some sort . ..it makes me work my body exercise in an infinite ways just to do my basic ADLs. Granted i do have some good ideas about healthy body mechanics.. letting my intuition guide me and my discomfort /pain to guide me. i use Couches and chairs furniture, countertops to modify exercise keeping Tara's conceopts in mind.

    I have gotten SOOOO many exercises from you, I would have to do it 24/7 to fit them all in. I try each one, and repeat the ones I can not do. As thats obviously a weakness.

    Thank you so much once again there is always so much to learn. Now it makes sense why the arm swing is also important, even though I was told dont worry about that it will never come back for you but I think that's wrong because if you do put some focus on it something might come back I think.

    thank u for so much information. my question as a self referred patient would be,
    what type of people you treat? apart from neuro patient, any one with pain with walking, knee, hip or SIJ dysfunction can also do these exercises ? or these are high level activities? does this require any basic level of strength. I mean older clients can do it easily

    Pretty intense, but good explanation. I watched it this morning before I got up, then later had to watch the exercises multiple times to make sure I was doing them properly.
    The hardest one was the kneeling exercise punching with my weak arm/hand.
    Also noticed that my weak foot wants to curl on the exercises where I use my strong leg to cross over (spasticity).
    Not too bad for the first time. Thanks!

      Just to follow up. So glad I pushed through it the first day (felt like it took forever), because today (second day) felt like a breeze compared to the first. Certainly hope & believe this will make a difference over time!
      Also, thought this was a great follow up to last week’s video on mobility hacks. It helped with starting to get more comfortable with getting on/off the floor & squatting from a low seat! 😎

    Hi Tara I found these exercises difficult on herma side as i cannot feel these muscles move, even after 5 yrs and exercising every day, so very tiring. I have Hip surgery on Left hip 2 wks ago, so still quite sore to move that area. I do the closed chain exercises in bed before i get up or if i wake through the night. Thanks Linzi New Zealand.

    Thank you once again Dr Tobias, although I am walking pretty well it's not perfect yet and I'm a big believer in whole side fascia connection or improving the whole affected side to improve both the leg and arm, though it is often easier to become hyper focused on what doesn't work yet in my case my wrist and hand.

      Hi TJ! This is a perfect video for those of you who are further along in your rehab. These "posterior chain exercises" can get rid of those little "quirks". I have been using these types of exercises (more recently) with patients who are in this category. A small "hitch" when trying to bring the leg through, or a little "jerk" mid swing. Getting that posterior chain activated in a standing position has been really helpful. But back to your comment, don't give up on that hand and wrist!! Keep working it. Follow "Neo" (aka "the_road_to_recovery_2020) on insta. He is so creative in getting that hand working!

    Hello doctor.. thank for this enlightening video.. was searching for something like this for months..
    I got upper back injury not in spine but near spine, scapula and shoulder region .. left side
    I started feeling bouncy while walking after that.. and off balance as if dragging towards right side right before my right leg hit the ground
    Not able to understand if that is due nerve or muscle
    Also if nerve which part of brain is affected right or left.. since injury in right side and leg issue in left side..
    I feel very less pain near scapula region..
    Plz help

    I am a stroke patient 2 small strokes in the right frontal lobe which occurred 20 years after brain cancer in that area in 199. I had successful brain surgery, chemotherapy and radiation to that area. No reoccurrence. I’m experiencing weakness in the left leg with tightness in the left gluteus Maximus and Minimus. Also tightness behind my left shoulder. So I’m confused why my left shoulder is trying to compensate for my bad leg. According to this video my right shoulder should be helping. Can you please explain. Why this might be happening. I am taking physical therapy for the issues and have been in therapy before but recently the left leg experienced weakness after wearing a walking boot for 12 weeks for a broken bone in my left foot. I would appreciate any advice.😊

    Thanks for another great video. Just wondering… are kinetic chains and spiral lines the same? Related? I never knew “lines” crossed the body until a pt did some myofascial release on me and explained spiral lines. And I am extremely tight on my upper body nonaffected side. Can this tightness contribute to walking difficulties on my lower affected side?

      I think the two terms have many similarties. Here is an article I just found. basicmedicalkey.com/the-spiral-line/ There is a soft tissue component beyond just muscle, however, I chose to only focus on the muscle component in this video. I hope that makes sense!

    My physical therapist told me the only way to fix an abnormal synergy pattern said the only way to fix is to have a baclifin pump. I’m not doing that

    I got really confused which leg was the weaker leg during the exercises, it seemed to change through the different exercises, Also as someone with not great balance and a weak left arm and leg, how I would hold on a stick for example and pull a tension band with the same hand at the same time?

    I have been struggling to find answers to what has been going on with my gait and feet. I have seen soo many people trying to find out what is going on and I even ended up having surgery on my right foot(tarsal coalition), and I am still having issues and have no answers. I am pretty sure now that I am overpronating causing flat feet. Not sure but thinking it could be related to something in my hips maybe(hip flexors, TFL, glutes, etc) I am in desperate need of help and some answers. Do you have any ideas of what I could do to figure this out.

    I will have hip replacement surgery next month cause my mobility is so limited, I do walk with short steps with that leg so how can I think while walking after surgery? cause my posture is weak and my upper body leaning forward while walking ..

    I love love love the posterior chain exercises. I couldn’t do the exercise where the stronger leg was across on top of the weaker leg while doing the bridge . Me ankle rolled and both of my legs fell to the side. I was able to do all the other exercises. It felt so great.I would like to ask how to get over being bothered everything being on my left ( involved side) while I am walking. I am walking without any assisting device inside my house, and use a walker for walking outside. I barely touch the handle of my walker.

    I walked normally but my leg runs out of energy after about 5 hours what causes that and then the leg just doesn't want to work

    When I’m walking I don’t know what to do with my affected arm. It’s either tight against me or it’s hanging there, dangling I can’t get it to look normal 😑

    The sole of my feet is very smooth, when I walk I tend to till forward like a baby. What can I do to change that. I have NMO and is trying to recover from my 3rd attack.

      Hello Kim! I also have NMO. Good to know there is a fellow NMOer here. I was diagnosed in early 2013, but it took months for the answer to come. First acute attack in September 2012. Rituximab twice a year. Tara Tobias has helped me as much or more than anything. I’m so glad I found her about a year ago. I continue to make gains.
