If you are trying to conceive, the fertility medication known as clomiphene citrate or Clomid may be an option for you. Clomid is used in two circumstances. First, Clomid is used for women who don’t ovulate to try to induce ovulation. But clomid is also used for women who already ovulate to try to further improve their chances for getting pregnant.
The protocol for treating women with Clomid varies from doctor to doctor. I am going to tell you about the highly effective methods we use at IVF1.
First, it is important to perform a baseline ultrasound of the ovaries and some baseline blood tests. Women who get their periods can do this on the 2nd or 3rd of menstrual bleeding. Women who don’t get periods can do the baseline at any time.
As long as everything looks okay, a woman may then start to take the Clomid pills. The starting dose is usually 1 to 2 pills a day for five days. Women who don’t ovulate may use a higher dose up to 5 pills a day or use them for a longer period of time such as 7 or 10 days.
After the last pill is taken, it is time to start monitoring for ovulation. With clomid, monitoring can be done by using a home ovulation predictor kit which allows a woman to test her urine every morning. A positive result means ovulation will follow in 12-24 hours.
Another popular method for ovulation monitoring is to have it done at the doctor’s office. Eggs mature inside of a cyst known as a follicle. As the egg matures, the follicle grows larger. This can be seen and measured on ultrasound. The cells that make up the follicle produce estrogen that can also be measured using blood tests.
After the last pill of clomid is taken, follicle ultrasounds and estrogen blood tests can be performed every few days. At times, the eggs will reach maturity without the body triggering ovulation. To get around this problem, doctors will instruct the patient to take an injection of a medication called hCG. Thirty six hours after the hCG is given ovulation will occur.
About a week after ovulation was predicted, another blood test called progesterone can be drawn to confirm ovulation. A week after that a pregnancy test can be performed and then -- fingers crossed.

Zaytoonah Fuzurally I took first pill on my 3rd day of period. Took pill for 5 days. I ovulated 5 days later , I had intercourse everyother day during this process and got a positive 2 weeks later.

I recently had a postive PT after taking clomiphene and letrozole… went to OB right after the +PT but no gestational sac was seen yet. I was given heragest and vitamins and advised for ff up after 2 weeks. Unfortunately, i bled and had miscarriage 3 days before our scheduled ffup. I used fermara for 6 months.... no luck. Now this is my first dose of clomid. I had 2 IUI's back to back. I am a week after the iui, so hopeful it will work. It was the first time in a year my ovulation stick turned blue on both lines!!
Update * it didn't work! I'll try another iui with Clomid in November

What causes a women to have regular period while on oral birth control? I’ve been off birth control for 18 months and trying to conceive and still no baby. I’ve never missed a period even while I was on birth control. I’m now trying to conceive and wonder if Clomid will even work since I assume I’ve always been ovulating due to regular periods. If oral contraceptives stop ovulation then why is my period regular and predictable? Is it possible to have regular periods without ovulating? When in the menstrual cycle do people start Clomid?

Hi Dr I'm trying to conceive since last two years, everything is normal according to drs,last cycle I took clomid then I had a lower back pain for almost a week around my period date, finally I got my period 5 days later of my actual date n the flow is very low .Now it's my 3rd day of period plz tell me , should I take clomid again for this second cycle or not? Hi, would you recommend using clomid with a .19 AMH? My ultrasound showed multiple follicles present and on the other side recently had a resolving follicle. Could Clomid help me or help with a low AMH?

How successful is oral medication like clomid or femara with gonadotropin injections?
Oral medications such as Clomid or Femara are occasionally combined with injectable gonadotropins. There are a variety of reasons why doctors may recommend this but mostly is is an attempt to get the higher pregnancy rate of gonadotropins while trying to reduce the cost (gonadotropins are much more expensive). In general, the pregnancy rate are a little better than oral medications alone but lower than injections alone

Clomid should only be used under the supervision of an experienced fertility specialist. There isn't a "one size fits all" Different patients will need different things. See your doctor!

At IVF1, we like to do a baseline ultrasound before starting fertility meds. We will not start if there are large cysts, multiple cysts or hormone producing cysts present
