Honestly, one thing I really wish the scenario creator had (that it might if it's been updated, haven't used it in a long time) is the ability to give one disease the *major thing* of another disease
like a neurax worm that can evolve to spread its tendrils throughout the whole body and puppeteer it after death, making zombies. or a vampire plague that can also cause full on mind control. and so on
I've been making a werewolf plague for a long time now. It's a Bioweapon because I want exponential lethality, but I also want Simian Flu active abilities and to be able to control and infect canines (which would work like apes), so this would be super useful.
Drug resistance could be done with an effect like "Conspiracy Theories: more people start believing medicine is evil"
