Total Hip Replacement at 19 Years Old

    I had my right hip replaced in the summer of 2009. Unlike most individuals who undergo the elective surgery, I was only 19 at the time. I had been suffering from progressively worsening hip pain over the previous two years that made it impossible for me to walk even short distances without a very noticeable limp. A combination of the pain, embarrassment of constantly being questioned by my peers about my awkward gait, and the encouragement from my parents led to the inevitable surgery between my freshman and sophomore years of college.
    The reason for having my right hip replaced differs significantly than the vast majority of people. I had a condition known as Avascular Necrosis which is essentially bone tissue death due to a lack of blood supply. AVN may be caused by trauma, excessive alcohol consumption, or other conditions, but it is most commonly seen in younger, active populations secondary to corticosteroid use. Cases of AVN have been reported from both short and long-term usage of corticosteroids across various applications. However, the exact mechanism behind this condition is unknown and it is likely to be multifactorial in nature. Unfortunately, a high oral dosage of corticosteroids for 3 years to combat a separate medical issue likely led to my development of AVN.
    “Will I be able to lift weights?” It’s one of the few questions I remember asking my surgeon. He told me that I would be fine squatting up to 135 pounds because there were individuals much heavier than me who put that amount of stress on their implant with their normal day-to-day activities. That made sense to me. He told me that I could be active like a healthy young adult. He wasn’t encouraging or expecting me to go out and play tackle football, but I think he was giving me the freedom to be responsible with my choices. We also talked about some of the risks associated with high-impact activities. The only thing I distinctly remember him telling me to avoid was bringing my right knee beyond my left shoulder to minimize my risk for dislocation (which I can’t do anyway).
    Here’s a short list things of things I’ve done since my total hip replacement:
    - Hiked all 22 miles of Mt. Whitney (14,505’ elevation) in one day.
    - Wrestling and boxing
    - Basketball, soccer, flag football, softball, ultimate frisbee, tennis, volleyball
    - Yoga, paddleboarding, skateboarding, ice skating
    - Deadlift 2x bodyweight for 10+ repetitions
    - Bulgarian split squat 185 pounds for sets of 3
    Are these amazing feats of strength, skill, or endurance? Not for many, but it’s a glimpse into what can be done with an artificial hip.
    Were all of these activities pain-free? No, but the discomfort of repetitive hip flexion as I ascended Mt. Whitney was worth the feeling of accomplishment at the top.
    Do I still perform all of these activities? Yes and no. I’ll happily play sports on occasion, but I can’t bring myself to be as competitive as I used to be. I don’t jump as high for a rebound or run as fast to catch a frisbee.
    I’ve decided that I’m going to lead an active, healthy lifestyle, but I’m going to do so while keeping my future in mind. There’s a lot of uncertainty about the longevity of the implant. If I can potentially extend the time until I need my first revision by minimizing high-impact activities, that’s what I’m going to do. It doesn’t negatively affect my quality of life.
    I am not recommending that individuals with a total hip replacement perform any of the activities mentioned. I am simply describing my experience over the past 11 years.
    Should you return to high-impact exercise? I think that’s a discussion to be had with your surgeon, physical therapist, and family. Is the risk of possibly needing an earlier revision worth the 10 years of marathons in the prime of your life? You might say yes. As I mentioned earlier, it’s not worth the risk for me anymore. My goals have changed and I’ve come to accept that.
    If you had no goals in mind and just wanted to be as healthy as possible, I would recommend a wide range of exercise: walking, cycling, hiking, resistance training, etc. Most people are not meeting physical activity guidelines as it is and it’s one of the healthiest things you can do for yourself, so the last thing I want to do is discourage people from exercising. I train hard consistently while staying on the low-impact side of things with very minimal pain.
    Can you return to high level activities? Absolutely. Should you return to preoperative amounts of high-impact activities? It’s not a black and white, yes or no answer. It’s grey. It’s complex. I don’t think there is one right answer. We’ll see how my hip is doing in another 11 years.

      @@sreyachatterjee5377 So it's been 8 months almost exactly. Feb 19th and I can walk great do basically everything I use too. I'd say the only thing that bugs me is when I sit down with my legs crossed or try put my leg over the other knee.
      But when you say "normal" after an Accident.. that depends on the accident. Cause my replacement was a graduale pain that slowly wore away. It's not like mine was damaged from a impact or break.

      @@trigger3319 still sore 50% of the time, but much better that before surgery. Sometimes It feels completely fine tho too. Cold weather or rain actually makes a difference as well. Other than that it's pretty good. I'm a pretty active guy so it definetly gets sore, but when I'm relaxing for a few days i feel great.

    I wish I saw this when I had my hip replacement, it really would've helped me. I got my hip replacement when I was 17 due to a cyst that made my bone really weak. First surgery failed so they had to go back in for a second surgery. I fell into depression after spending 2 years unable to walk but im glad to say I made it through, and I'm glad to hear so did you.

      I also have a cyst which I didn’t know about so when I got hit by a car the impact caused it to fracture and I haven’t been able to walk in 9 months. I feel you bro

    I had ALL cancer at 18, I am 21 now, I have AVN, I am not in serious pain but I am going to get the THR anyway, I don’t see why I would wait, I hope to get back to weightlifting asap, thanks for all your videos the internet is truly a blessing

    Thank you for posting this! I’m a 40 year old retired power lifter and just had THR last year (Jan 2020) on my left hip. It’s nice to find information from a doctor on what I can still do when it comes to working out post surgery!

    Good evening. I was diagnosed with avascular necrosis both left and right of my hip joints. I pray that the operation will be successful. I am experiencing this struggling since 2012, so it's been 20 years now. 🥺
    I can't stand in a long period of time but 15-20 minutes then the pain starts and I can't walk again and feels like my hip joints are locked and I can barely move my legs. I miss playing volleyball, flying saucer, taekwondo, kyokoshin, swimming and running. But for me, once the operation done, it's ok not to get back to those activities as long as I can manage and enjoy my life in some light activities that still can make my body healthy and functioning especially my legs.

    Great video, greatly appreciated. I Myself got a thr in my mid 20s, and will be getting my other one now in my mid 30s, I just wish i would have got the 2nd one done sooner to get back my rhythm in my life since i have thos other hip holding me back right now. Condition was also AVN of bilateral hips. 1st THR made a great change in my activity. Cant wait for my other one, In the name of the father, Im hoping to feel less restricted in what i can do and achieve since my hip pain has always restricted me. Blessings to all going through this life battle, A briggt and sunny day is ahead of us.

You give me the approximate duration of hip replacement and understand the time and adjustment that one need to make before you can function again. At first I want to have hip replacement now but I just realized that I should wait a little longer. Hip revision is more complicated as well.

    OMG Thank you so much for this video. I got into a car accident at age 7 and have a fused hip since then, now I'm 30. There's no pain or anything, I just limp a little and feel uncomfortable walking and this basically ruined my whole life and took all my confidence away, but I'm doing boxing and wrestling and basketball, so I was afraid I won't be able to get back to my physical activities after the surgery and that's why I still haven't had it. I've been asking these question for a long time but nobody gave me a good answer. Thank you so much and I wish you all the best in life, I'm going for the surgery.

      If you have no pain then don't go for surgery better modify your shoes sole by inches so limping will reduce .

    Hello, Because of this vid that you made.. I have just subscribe, and I want to learn more. You seem very knowledgeable and that is a great thing due because of our hip conditions. I am 33 in the month of August. and it just gives me hope to see someone like you with a HIP replacement and that you can still lift weights. I have hip Dysplasia since birth as a birth defect :-'(

    I was searching young age people who did this surgery, and found this video. Thanks a lot for sharing your experiences. I am 30 years old and I had congenital hip dysplasia, 12 days ago I had my total hip joint replacement surgery. Surgery was good, I still have fears though. 6 weeks after the surgery physical therapy has to continue. Doc said total recovery process is 3-6 months.

    Thanks for the video, nice tips in there. I have had 3 total hip replacements. First one when I was 20, then a year later for the other hip, then another year later I had to get the first one redone! So anyway I waffle. Now over 20 years since then i am still here ;)

I'm so worried about what I will and won't be able to do if I move forward with the surgery, so it's great to see that you are still able to be very active even after your hip replacement.

    Thank you for this, it really helps to hear about a case in a young person. I've had problems with my hip and knee since some car accidents when I was 20. Now, at 32, I had another MVA last year and my hip is getting so bad that I walk with a cane and can barely go on my walks that I love. The prospect of a hip replacement while I'm still a single, active person seems scary, but this put my mind at ease a bit!

    I'm 31 years old. I had a football injury when I was 13, posterior L hip dislocation. I had arthroscopic surgery a year later that revealed a full thickness cartilaginous defect which has caught up with me. I am scheduled for surgery on Dec. 8th. Watching your videos and reading comments of other young people undergoing this surgery helps big time. Hoping and praying for the best moving forward. Thank you!

    Thank you for making this video. I'm 48, 6 weeks post op.I was very active prior to my surgery and did a lot of weight lifting. I was beginning to feel discouraged but after watching this I have hope. I'll be able to do all of the things I loved doing prior to surgery:)

    This is super sappy but I'm about to go squat and I want to say how grateful i am to you and this video. I was diagnosed with avascular necrosis last summer and fought to have surgery (i was apparently "too young" even though it would impact my quality of life during my "youth"). Powerlifting is the closest thing i have to a spiritual practice and to possibly not be able to do it anymore made me crushingly depressed. It was bleak. One day I stumbled across this video after trying to learn more about what was happening to me and what i could expect. I had surgery last Nov. I know i'll never be able to hit my old PRs (or at least it would be extremely ill advised to) but i can deadlift and squat again and for that i am happy.
    Thank you, thank you, thank you for making me feel like there was hope.

    Thank you! This video is inspiring to me. It's both hopeful and realistic.
    I'm about to have my right hip replaced on August 2. I'm on an 90 day sabbatical now for various reasons including so that I can work out every day and train up *before* the replacement with functional movement using TRX (with a good trainer), Bikram Yoga (correctly), Orange Theory (with a sane trainer), swimming, hiking and some weights.
    I've never been as athletic as I would like to have been, and it took many years to discover that I had so many problems with this hip, from an early labral tear, to possible nerve damage, to osteoarthritis and finally disappearance of most of the cartilage. This re-enforced all kinds of bad habits, and actually I never had learned good ones.
    My hope is that I become more active than I ever was. I have no interest in anything high impact, just correct movement and everything I can do with that. I was never able to squat more than 135 correctly because of problems I never knew I had until much later. I am so looking forward to this!

    As an active 63 year old, I just had my left hip replaced 10 days ago and the joint pain is completely gone. I feel 100% better.The only discomfort now, is the muscle soreness from the surgery itself. My surgeon used the Anterior approach which seems to be the better option for a variety of reasons but not all surgeons are up to speed on this method.
    There is nothing to fear with regard to the surgery

    Thank you Doc for this video. I am glad this was shared in our group. I am in search of resources to help with my battle against AVN in my left hip. Six months ago I decided to take personal responsibility for my health after two years of struggling with the disease and i have worked my way back from walking with a cane daily to biking 20 km 5 days a week and a full weight/core workout weekly schedule. My hip is 90% better than it was when i started and I have recovered almost all my range of motion while reducing my daily pain meds drastically. For me it has been a lifestyle changing decision. I feel great and at 49 I am probably in the best shape of my life. After joining a support group I realized just how lucky I am. But I can't believe I am the only person diagnosed with AVN living like this. I am looking for like minded active people to share experiences etc. Doctors recommend i try not to do anything and the only course of action is a replacement. Being the rebel I have always been I am intent on proving this to be an option. The medical advice in Mexico is not trust worthy when you are a foreigner so support is important as i feel very isolated.

      I was diagnosed with AVN of my other hip at the same time, but supposedly it was less severe. I was close to getting it replaced 6 years ago, but held off. I have very limited range of motion, but I'm able to do most of what I want without pain. I do my best to maintain a healthy weights while having good exercise and dietary habits. I can't give you specific medical advice, but I know that I personally do my best to live a healthy lifestyle.

      @@E3Rehab I am very lucky I believe. To be able to do as much as I do but also my opposite hip is healthy and after two and a half years they tell me there is very little chance it will develop in that hip. I would like to know if you are aware of any forums or support groups other than your resources for more active people with hip issues. The current ones I am part of while helpful are not really full of like minded people. It adds to the depression of someone trying to stay healthy mind body and soul if you understand what i mean.

    I am 47 and scheduled for my first hip replacement. I have bone on bone and have lived with the pain since 2006. I now can not reach my foot to tie my shoes and have constant back pain. I can walk but if I walk too far I inflame my hip and and am laid up for a few days with a steady diet of anti-inflammatory meds that tear my stomach up. Thank you for this post and the blog. I am nervous since once you go this route there is not backing out and I do not want to put myself in a more difficult situation.

    Thanks for your video. I am very grateful for that. I had my total hip replacement 11 days ago. I had osteoarthritis due to femoroacetabular impingement. I'm 36 years old, I'm Brazilian and I'm passionate for soccer. I used to play 4 to 5 times a week, but 5 years ago I started to feel pain in my right hip, until this year I had to stop to play due to the intensity of the pain. It was also impacting in my walking, sleeping and normal daily activities.
    After the surgery, I had a lot of doubts about what I could do in terms of activities, especially because I have 2 small children and I always played with them. I was a little sad, but after watching your video I started to feel encouraged and motivated.
    Again, thank you very much for that and please continue to post videos on the subject. It will help, encourage and motivate many people around the world, like me.

      I'm not playing yet. I'm still doing strength, mobility and stretching exercises. Also, I'm swimming twice a week and bicycling.
      I will back to the doctor in two weeks and hopefully, I will be allowed to play soccer again. Let's see.

    I’m 23 and I was diagnosed with AVN in both of my femoral heads - Feb 2023. I went to the hospital with severe poking pain in my Right Hip and after MRI it was confirmed that my Right Hip was stage 3 and Left Hip was stage 2 of AVN.
    My doctor suggested to not go for the THR considering my age and instead recommended for Bilateral Core Decompression of both my hips and BMAC as well.
    Now 10 months after surgery and could feel the difference. Slowly getting better and hoping for the best.

      Do not follow this video, THR patients can't lay down or sit on the floor forever. Better check with your surgeon. They make videos just to get high no of views. I recently had THR done on left side and i have been asked not to sit or lay down on the floor.

    My friend had a hip replacement at age 55 due to a shattered and splintered and crushed hip because of a fall. He started walking on uneven outdoor terrain 4 days later without aids.

    Hey I’ve just found this video! I had my hip replacement two days ago at the age of 16. I have the exact same condition (vascular necrosis) caused by steroids in a previous illness. I was on crutches before my surgery for 1 year but luckily was pain free. I want to be as active as possible so thank you so much for this video explaining into more depth! The doctors said to be really careful for 6 weeks as i’m very prone to dislocation due to being young and having really speedy recovery. So let’s hope i can return to activities I love (hockey, tennis, cycling, yoga, gym) 🤞🤞

      I’m 20 and had mines replaced two months ago for the same condition. Second year in college and I was a athlete all my life. I knew I wasn’t going to get back to track and field how I was use to but i jog a lot now. Glad to see other young people around the same age ( we’re 4 years apart but same thing 😂 ) having great recoveries.

    38 yrs old and having my lft hip replaced next week and I'll have my right hip replaced soon after. I tore my labrum and several muscles originally and then arthritis developed. I live with constant pain in my hip and a tearing sensation in my groin when I walk, and with nerve and muscle pain down my legs. Thank you for the inspiration.

    I too had a hip replacement at 16, and now at 17 I'm getting my second one. It was due to corticosteroids for a cancer treatment. And my function was very bad and it was 100% needed. Thank you for sharing your experience!

    Good video and thank you. I’m 56yo and 6 weeks post op at with my second hip replacement and just watched your very good and informative video for a bit of peace of mind and refresher. I’m sorry to here about your health problems in your younger years but I’m sure your family would be very proud of you by becoming a doctor with the first hand knowledge about hips that you have. Not to many doctors talk about pain post op, but I’m glad you put it out there to let people know.

    Thank you so much for this video! 😭 I’m 24 & was diagnosed with AVN too! My total hip replacement was 6 months ago & I am in distraught that I can’t squat heavily or go to the gym as usual as I wanted to. Depressions been playing with me & it SUCKS. I’ve been trying to slowly start again, but like you said it doesn’t happen overnight. Again, thank you so much for these videos!!

      Vinna, I know I’m the “most recent” recipient of a double hip arthroplasty and I was very skeptical at first. (Meaning will I be able to do the things I want to do like play basketball, tennis, lifting and riding my bike.) I’m actually more focused because it’s all on me. My doctor said don’t get hung up with not being able to do certain activities. For instance it’s been 5-6 years that I’ve ridden a bike because of the pain it caused me. So, it will be a long process but I’m up for it! When I got on my bike a month ago and rode pain free that’s when I said “I need to get the other one done NOW”! LOL! Glad I did and now it’s all about rehabbing daily. Having a positive group of people you can talk to also helps a great deal. Hold that head up high! We need to start an online support group for those who have had hip replacements. What do you think? I’m on IG @Robertho1 & @Allmyphotosrmh

It’s not just simply exercising but it’s a part of our daily life & it can be so crucial to have it taken away & nothing we can do about it. But yes! You must get the other hip done ASAP. I had both mines done back to back & the recovering was not pretty .. praying for your fast recovery 🤍 Stay strong, we can do it 😊

      Vinna Lor I actually did get the left one done!! So, both are replaced and I feel like a new man!!! I know there’s the recovery time of up to 1-2 years. I’ve changed my way of doing things and I’m cool with it! I’m riding my bike already after 1 week of having surgery so I’m blessed. Wishing you good luck with your double hip replacement too!!! We’re like a tag team!!!! LOL

    I just had a hip replacement on the 1st of July due to a large cell tumor in my femoral head and actually broke my femur at the neck on the 30th of June when I slipped with my crutches going up the stairs and put all my weight on my left leg. I greatly appreciate this video since I'm only 22 and I was scared at first to get this procedure done. Ended up not really being a choice after my incident but I greatly appreciate your perspective and your story. Thank You so much bro.

I actually had some difficulties in the last year - replacement has been three years ago - with my replacement because of muscular weakness, but was able to work it out together with my orthopaedist. Now I am back and can be active again.
      Still not regretting the replacement, since even with the problems, it was better than before.

      I am fine so far. I have some problems with my replacement because of my specific medical history, but learned to live with them. My daily life is still a lot easier than before the replacement and I am optimistic it'll last for another few years. I am sometimes still amazed about being able to walk more than a few meters pain free even though it's been a few years since the replacement.

    Fellow AVN hip replacement here. I got mine done at 34 years old, going on 3.5 years now. Back to most of my pre-AVN activities but I too chose to limit and avoid some activities to try to extend the life of the implant.

    Hi.Enjoyed your video Thankyou.I do have one question I'm 63 I've had both good done.I used to train 11 times per week.2 boxing sessions, 3 Shotokan Karate sessions and 6 weight training sessions. I also did 3 to 4 6 mile runs per week just for enjoyment. I've never stopped the weights training in 39 years.I've been out of the boxing and Karate for 23 years.Ive started stretching about 3 months ago hopeing to go back to teaching Karate .so that's my background. My question is;How far in the box splits will these hops allow me to go.Thanks in advance

    I was diagnosed with AVN that came from taking prednisone as part of my treatment for a kidney disease I have. I had it in both hips and had them both replaced at the age of 27. Within 6 months it progressed so fast that I went from having total independence to limiting my activities to only work and doctors appointments. I couldn’t even bend my hips enough to sit in my car or even put my own socks on. It was the worst pain I’ve ever felt. Getting both of them replaced was a no brainer. I was also confirmed to have AVN in both of my elbows which sucks because elbow replacements aren’t an option for young folks. It’s been a year since both of my surgeries and I have returned to an almost normal life. I do have minor aches that make sleeping hard sometimes but otherwise no complaints.

      When did you find out AVN in both elbows? I am so sorry! I have AVN in both hips and got them both replaced, but I only had iv steroids for 5 days. I was told you only get them in one place and it does not spread. How did you come to know of AVN in both elbows? How far apart from your hips were you diagnosed?

    I’m 32 turning 33 - I had leg-calve perthes disease as a child that resulted in 3 surgeries later with a leg that is almost 2 inches shorter and a life full of pain, “toughing it out” and pain in other areas in my body. I need a hid replacement and I have been “putting it off” afraid of how it’ll change my life. Thank you for sharing it.

      Had perthes myself as a kid. I wore leg braces for three years. I'm scheduled for hip replacement in a few weeks at 43 years old. My bad hip was causing lower back and knee problems as well.

    I had a total hip replacement for both hips when I was 12 years old. I have hip dysplasia.
    I just want to strengthen my leg and hip muscles. I'm so tired of feeling like "I can't"
    There is so much I never got to experience as a kid because of the pain and as a teen because of the surgery and recovery. I let this effect me and I was scared to push myself. Now I'm 29 and not sure where to even start. My hips are pretty week with limited range and its discouraging at time.
    This is the first time speaking about my hip replacement and it feels great to know that others are or have gone through this as well..Thanks for the support ❤️

    Did you look into Birmingham hip resurfacings at the time of your replacement?
    What kind of replacement do you have exactly?
    Were you told about dual mobility hips? Do you know much about them and if they've got any advantages over a traditional THA?

      @@SkyHermit so I got diagnosed with Avascular Necrosis at the age of 27 and got my surgery done at 31. Doctors have no idea why I got it.

    Im 35 and had a hip reconstruction/ FAI procedure about 7 years ago. Been in pain since age 13. Now im considering a replacement but i dont think im old enough, but the pain has returned for the long haul. But im sick of the throbbing and burning. I am able to work but its all i can manage to do now. I dont do anything else. No more extra walking, biking, driving, sitting, social events, i order online vs going to the market, etc. I even turn down girlfriends when I'm in a painful mood because it makes me grouchy. I never ever commit to plans because idk how I'm going to feel and if Im feeling good my free time is dedicated to strengthening my right glute which has stopped firing. When im sore i just have to go lay down. Sitting down is not even an option. Mentally its really worn me down too, and my only goal is stacking cash to retire asap, however extra work just irritates it worse so i have to be very careful. I've also switched jobs almost every year because I get an attitude from the pain and I eventually get fired or self sabotage.

    When I was 7 years old I suffered an accident in my hip and now I am 23 and the surgery they did at the time of the accident was not so good and now after so long I have to have a hip replacement because my bone is damaged, I am nervous but seeing this video motivated me a lot that everything is going to be fine ...

    I was going across several videos, for information regarding recovery periods. I myself had 4 surgeries when I was an infant between the ages 1.5 to 4 due to congenital hip dysplasia. And 6 weeks back I had my revision Total Hip Replacement of right hip. I am currently in bed rest having undergone a 10 hour surgery the doctors have put me in a de-rotation hip cast. According to the doctor, it might take me a total of 14 weeks atleast to atleast stand up.
    After seeing the comments of this video, I am confident that I shall be able to come to my normal life gradually.

    Thanks for uploading this, I am 28 and have needed a replacement since I was 22. I've been putting it off but I feel like I'm reaching the point where I can no longer manage my pain. Im a little terrified by the whole process and the thought of not being able to exercise but more so running after ive had it replaced.
    What does it feel like to get some normality? I've never had a day in my life since I was 11 that my hip hasn't hurt I was diagnosed with suphe originally and now have cam and pincer impingements.

    I'm 29 and the other side of my femur was replaced. I had a distal femur replacement. I'm still working on getting back to "normal". I lift weights too. This has somewhat traumatized me since for so many years I felt restrained.

      @@lbenji13 yes I had mines I’m going on 4 weeks post on Monday . Had it December 21st . It was hard because I had the huge posterior one but the first two weeks are the hardest . After that it gets better day by day . I’m on a cane right now

    Thank you, i just returned from the Doc, I'm 52, 10 years post cancer, i was treated with steroids, Prednisone, i probably need a double hip replacement now

      I'm 26. The doctor said it will be better after I heal. I have to do both hips. I just did my left hip this week. They gave me a bunch of pain killers which are helping. I can't really move around too much right now.

    I had my core decompression done in 2018 due to Avascular Necrosis, I am fine now but my activities are very limited as I still get pain, Unfortunately I have on my both Hips. I am just waiting for a right materials that longs last to get a my both hips replaced in the coming future. I am 29 now.

    Thanks! I’m 40 and have Hip Dysplasia, I’ve not kept up with activity and “sunk” unto my seated office job. It’s pretty bad lately, even used a wheel chair at a recent art gallery cause I could stand for hours looking at art. Honestly I’m scared to have the surgery / this is helpful. I really thought after a surgery one couldn’t move your joint more than 90 degrees is that old news?
