This depends on a lot of factors and will differ from person to person. However, keep in mind that individuals who go to inpatient rehab often have more limitations, complications, and challenges, which is why the are probably discharged to an inpatient facility in the first place. So the short answer is it depends on your individual circumstances.
Typically you can start using these when the incision is fully closed and the tissue is given some time to heal. Usually I recommend to patients they can start doing this at about the 3 week mark, so I would double check and confirm with your surgeon and/or PT. They may have a reason they want you to wait longer.

Hi, I am 3.5 months post bilateral knee replacement surgery and have developed scar tissue across the pateallar area which is causing restriction when I'm trying to bend my knees. Will these techniques still work to soften the scar tissue???
These techniques over time have the potential to be beneficial for the superficial scar tissue from your incision. If you have developed deeper scar tissue or more advanced restrictions, you may have to utilizes other strategies. Typically the first line of defense with range of motion restrictions in intensive therapy and exercises working on knee range of motion bending and straightening.
Thanks for the reply. I am still in PT and doing stretching exercises. My flexion ranges from 110-113 degrees but I constantly feel the constriction across my knees. Do you have any information on shock wave therapy and it's value ??
No, lymphatic massage has a very specific technique and is usually incorporated with other treatment interventions. I would search for a certified lymphedema therapist.
Hi, Im 6 months post surgery knee replacement, im also 14 months post surgery after knee replacement my first Knee is great apart from a little stiffness but the most recent i still have pain on the left side of the knee and only have 80 degree of knee bend and also fully I cant fully staighten my leg , i did exactly the same excersise routine as the first knee replacement am I wasting my time now carrying on with the excersise and has the scar tissue now formed Thanks
Thanks for your question. I don’t believe it is a waste of time because it is still important to maintain good range of motion and muscle strength. There is still a possibility of increasing your range of motion but it will definitely be more challenging as scar tissue has started to form at this point. With consistent effort some improvements are possible.
I recommend wait until about 2-4 weeks after your surgery. This ensure that the tissue has some time to heal and you aren't disrupting the early healing process.
I had TKR a couple of months back. After surgery I developed huge hematomas up and down the leg which, for the most part, have cleared up. There does remain a residual resolving hematoma on the posterior part of my lower leg. It looks like the place the blood pooled post op. The stain does not seem to be resolving. I have tried massage at the same time as I massage the knee but there isn't a large change in the stain running from my heel up to the top of the calf. Quite ugly if I want to wear a dress where my legs show. Any suggestions or do I need to accept that this is permanent? Thanks for the great videos you have provided. Just what I needed to get motivated.
You may just need more time… most of the time this clears up but it just takes longer than we wish. The massage is good, you can also try elevating the leg throughout the day and ankle pumps throughout the day to try to flush it out, it can take a long time though
I had a ACL reconstruction and MCL repair, my scar is tuck to my muscle were it was harvest I can’t really move my skin , how long will it take for my skin to get loose after massaging ? I’m stress I been massaging for 3 weeks
The loosening and mobility of the scar will really vary and it is hard to give you a definitive timeline. I can take a very long time.. many weeks to months to notice an improvement. The massage helps but you still have to give the soft tissue time to heal.
I am going into my 4 th week post TKR. Thank you for all the explanation in regards to what to expect and how soon after surgery I am expected to achieve certain tasks--PatricianMcHale
I don't have a specific brand to recommend. There is potentially some benefit of coconut oil or lotions with vitamin E in them, but there is a lack of solid research to say these help diminish the scar tissue any more than just using a regular lotion.
I had right TKR 06/19/23. 7 wks Post-Op at this point, and my knee flexion is 90° degrees, unfortunately. I'm going to do the massages that you showed us, and hope to break off the scar tissue. Thank you so much.
Hope you see the improvements you need. The MUA can be a very beneficial procedure and often greatly improves range of motion and mobility. If you do end up needing an MUA understand that it will likely be very helpful for you!
I'm 6 weeks post op and I appreciate the in-depth analysis of these videos that my own therapist doesn't provide or I might not think of asking. Additionally, not everyone can afford extra PT sessions depending on insurance coverage. With KRT approach, it's like getting a free one-on-one session. Thanks KRT for taking the time to share.
A lot of those natural test things become inconclusive wink wink because the pharmaceuticals want their money! Great video! I still have the butterfly stickers on mine but I am definitely going to come back to this video many times for I do not want scar tissue which causes you not to have your full range of motion. THANKS For this video!!!😊
It is not a bad idea to direct distal to proximal to bring fluid out of the leg, but the goal of these techniques are really more focused on the incision itself and mostly likely won’t dramatically alter swelling and lymphatic fluid
Commentary is too long. In addition, the advice to use a lubricant does not align with other PT advice. More precisely, the use of a lubricant will impede skin movement across the fascia and reduce massage effect on fascial release.
Although there are sources who don't recommend using a lubricant, there are also plenty of other sources, and fellow PTs who I have worked with, who will utilize a lubricant. The lubricant keeps the tissue soft and moveable and is often much better tolerated by the patient.

Thank you for watching and commenting! I hope you find the video helpful throughout your recovery!
Please show how to do it with less talk
Thank you for your feedback, hope you were still able to find the video helpful!
You better do massage from down to up your knee, because lymph’s flow direction, otherwise its not healthy to run all down!
It is not a bad idea to direct distal to proximal to bring fluid out of the leg, but the goal of these techniques are really more focused on the incision itself and mostly likely won’t dramatically alter swelling and lymphatic fluid
Bottom line please
Thanks for the feedback

Thank you my hero
Commentary is too long. In addition, the advice to use a lubricant does not align with other PT advice. More precisely, the use of a lubricant will impede skin movement across the fascia and reduce massage effect on fascial release.
Although there are sources who don't recommend using a lubricant, there are also plenty of other sources, and fellow PTs who I have worked with, who will utilize a lubricant. The lubricant keeps the tissue soft and moveable and is often much better tolerated by the patient.
Yes, don't tell us what you are going to show us and why, tell us while you are actually doing it !
Thank you for the feedback 🙂
@@KneeReplacementTherapists And thank you for your video, it's the best instruction for scar massage that I' ve seen, will use it.
@@jstwntmusic you’re welcome!
Would it kill people demonstrating scar massage to do it on a real scar? Videos like this are like demonstrating how to play the accordion without using an accordion....
I agree there are certainly some benefits to demonstrating with a real scar after surgery, but I do think this video still offers some useful insights and advice. Thank you for watching and commenting.
@@KneeReplacementTherapists Oh, don't pay me no mind.... I have quite a lot of scarring from various escapades over the years, and I have found massage therapists consistently very reluctant to get stuck into massaging it. Even when they advertise that as their specialism. Like a lot of people, I watch massage videos for vicarious comfort, but the lack of IRL relief can make a chap grumpy.
This is good without a real scar. I dont see why you would need a real scar.
@@sharonwalton6129 You're quite right. And why are they using a real knee as an illustration? Why don't they just wave their hands around in the empty air? Or better still, not use their hands at all? And remain absolutely silent all the way through the video?
@@hugojames85 Absolutely. Silence is golden. However, they could AND should take this even further by telepathically transmitting this info to the viewers. 😼✌️
