    well i have been taught that the introduction must include the chief compliant and then the purpose. Is that correct?

      This is right but it doesn't have to be complaint and then purpose.
      As long as the reason you are writing is made clear in the introduction and throughout the letter, that will be enough.
      So, an introduction like this is enough - 'I am writing to refer Mrs X for further investigations into her suspected carpal tunnel syndrome.' This intro has 1. the purpose (further investigations) and 2. the presenting complaint (carpal tunnel syndrome) :)

      You cannot use your pen during the 5 minutes reading time. However, after 5 minutes you can underline notes.
