You can actually cheese the last phase. When is healt is close to finish for phase 2, wait ultil he is going to throw his sword to start the teleport combo, kill him, and wait in the middle. He will do one of the 2 animations (grab sword or attack with it) before going to phase 3, but now his HP is the one of phase 3. If he does the sword grab, you have more that enought damage to destroy him before he throws the sword (you can use the gem to +50% damage with max blood).
