The mercury used as a laxative is not all that toxic. There are two forms of mercury poisoning that are caused by 2 different forms of mercury. Methylmercury, [CH3Hg]+ (apologies for the lack of super/subscript), is the bioaccumulative toxin you're most familiar with. It's produced by certain microbes that live in aquatic environments, among a few other sources, which is why leaking metallic mercury into the environment is bad. It's also produced by certain old industrial processes, so a good chunk of what's already out there comes from the bad old days. Methylmercury is absorbed into the bloodstream by the digestive system. Our small intestine is set up to collect organic compounds really well, so it can catch the mercury by grabbing the hydrocarbon side. Metallic mercury, Hg, is the form of metal you've heard your parents or grandparents talk about back in the days when they let students put a bead of it on their hands. It's a liquid at room temperature, so it offgasses a bit because that's how liquids and gasses work (partial pressures and all that. Thus, you get fumes of metallic mercury. Metallic mercury is absorbed into the bloodstream by the respiratory system. Those fumes, if you breathe them in, get right down into the alveoli, and are capable of slipping over with the oxygen and a bunch of other stuff that you probably didn't really mean to breathe in either. These forms of poisoning actually have different effects on the human body, but I won't get into that now. The old mercury laxative treatment was distributed in pill form. Softgel pill technology was developed in 1834 and two piece capsules in 1847, so it's likely mercury was delivered in this format. (Aside from which, excipients are apparently meant to be used with solids only, so mercury wouldn't be suitable for that type of pill.) Since the mercury is in an enclosed capsule until it reaches the stomach, this means it has very little chance to fume up into something that winds up in your lungs. And as metallic mercury is not absorbed through the digestive system, it's safe and effective as a bowel cleanser. (Basically all it does is it puts a weight on top of the blockage that, being liquid, adjusts to fill the shape of the blockage, increasing pressure and, ideally, pushing everything out.) There's two reasons this is inferior to modern oral laxitives. One is that increased pressure is kinda uncontrolled. If the blockage is lodged in place tight enough....let's just say that failure always happens at the weakest point, and if that point isn't the way out, you've now got a much bigger problem. (Other laxatives act by weakening the blockage such that it breaks up at normal pressures, rather than pushing harder) The other is that dumping mercury into the environment is something we try not to do, because it gets eaten by the microbes that turn it into methylmercury, and now it's everyone's problem. It also may be worth noting that I don't know how easy it is to tell the difference between these forms of mercury, or indeed if they knew that distinction was important at the time. If they didn't know it needed to be separate, or had a poor means of ensuring that separation, it's all but certain that the mercury pills were a mix of both types, so you'd be getting some toxic mercury and some non-toxic mercury.
