Cleft Lip and Palate - Pathophysiology, Causes & Management

  • Cleft lip and palate Is one of the most common congenital facial defects. The incidence of cleft lip and palate is approximately 1 in every 800 births and affects males most often than females. A Cleft lip is a congenital appearance where there is an opening in the upper lip. Cleft Palate on the other hand is a congenital situation where the defect or the opening exists in either the hard or the soft palate or both. This creates a communication betwen the oral and the nasal cavity. Cleft lip and palate can either be complete or incomplete. A complete cleft lip is a type of cleft lip involving the lip along with the nostril, however, an incomplete cleft lip spares the nostril and is limited to the lip only.
    On the other hand, a complete cleft palate is a cleft located at the entire length of the palate affecting both hard and the soft palate. While an incomplete cleft palate is a cleft located on the posterior side of the palate or at the soft palate only.
    Both the cleft lip and palate can either be isolated or in combination with one another shown here in this diagram.
    These Facial clefts arise from incomplete closure or maturation of embryonic processes,
    A cleft lip arises from the defective fusion between the medial nasal processes. While cleft palate results from failure of fusion between the palatal shelves.

      Just wanted to update you on my granddaughter's Cleft lip/mouth situation. As I've commented before my granddaughter Michiru, pronounced (me-chee-roo) was born on September 17th,2022cwith Cleft Lip/ Mouth. Well after better inspection on her situation, only her lip and 3-4 teeth gum are affected by the Clefting birth defect. She will only need 3-4 surgeries in her life. The 1st 3 surgeries will happen between now, and 1st or 2nd grade or age 6-7 yrs. old. And her 4th surgery if needed around age 16-18 yrs. old after wisdom teeth come in. Anyways, Michiru's very 1st surgery as a baby has been scheduled for January 11th, 2023, just one week shy of being 4 months old. Please keep her and her family in your thoughts and prayers.     what i've learned from this video:
    1- cleft palate is a congenital situation where the defect exists either on the hard or soft palate or both
    2- cleft lip and palate can be either complete or incomplete
    3- cleft lip arises from the defective fusion between medial nasal process along with maxillary process
    4- cleft palate results from failure of fusion between the palatal shelves
    5- most common cause of cleft lip and palate is hereditary and the main problem is within genes where the defective genes are transferred from parents to their offspring
    6- there are pregnancy related causes causing cleft lip and palate as smoking or drinking alcohol in a high amount during the first trimester of pregnancy
    7- other causes of cleft lip and palate can be: folic acid deficiency, diabetes or taking certain medications

    My daughter's little girl was born this morning on Sept 17th, 2022, and we found out she has cleft lip/ possibly both cleft lip/palate . Having this explanation, is helping me how to understand how to care for my granddaughter when I'm needed for babysitting by my daughter.       I just stop by to send you my best wishes to you, your family and your granddaughter.
      She must be 1 years old now and had some surgeries already done. The technology now is much more advanced and the need for multiple surgeries like I had until I turned 18 is much lesser.
      My photo here is from my self and you can barely tell I have a scar from all those surgeries 20 years ago.
      I was born with both cleft lip (bilateral) and cleft palate involving the hard palate.
      But I would say that the care I had from my family, later on the friends who never ever made me feel like I had anything was a huge factor in not carrying when people stare.
      She WILL be fine and happy and she will deal with other stuff in life, like other people do.
      My grandma passed away in the 90's but I have lovely memories of her taking care of me, crying every time I would leave for a surgery, at the time when it was believed that kids born like this will have difficult life or even not survive. That is completely false.
      The scar she will have will just be a part of her and she will go on to face other things not even thinking about that.
      You can love her and hold her and help feeding her, but the bond she'll develop with those who care for her will prevent abandonment issues or insecurities in forming healthy relationship with others.

    Children with a cleft lip also can have a cleft palate. The roof of the mouth is called the "palate." With a cleft palate, the tissue that makes up the roof of the mouth does not join correctly. Babies may have both the front and back parts of the palate open, or they may have only one part open.
    Children with a cleft lip or a cleft palate often have problems with feeding and talking. They also might have ear infections, hearing loss, and problems with their teeth.

      No one is sure how it happens, if it’s environment or genetics or both. No one really knows for sure, all we have is guess work.

      The etiology/cause is multifactorial so we can't exactly attribute it to just one thing. It is not clearly genetic or hereditary or caused by folic acid deficiency or substance use. These can all be risk factors that may contribute to the probability of cleft lip/palate but it's not a linear relationship.
