The Chemical Safety Board (CSB) did an detailed investigation of the Deepwater Horizon disaster. The CSB made a very interesting video to summarized their investigation! The key safety equipment that could have prevented this disaster, the blowout preventor was not tested before it was installed on the ocean floor 10,683 m below the surface. If the blowout preventor had been tested, the owner of the well would have found out that the blowout preventor had several flaws that render it totally useless! The Deepwater Horizon disaster is a huge failure of the management systems!
