Hi It's better to change your name into S kavya in Bank passbook and take seal and signature from manager. If that Bank didn't make those changes, then open new account in any other reputed National Bank with the correct name. Have a great day.

Hi Raghavan It won't become any problem As initials are used for surname and middle name only . Name is not different in passport and certificates As in certificate we give names with initials, but in passport we will expand those initials. So no problem it's perfectly alright.

Hi As per matriculation certificate (10th standard marks card) write your Sur name according to that only So now Surname will be Kavya Or else you need to make affidavit for the name change , then need to publish the name change in local news paper and in 10th marks card , it will be hectic procedure to do so . Hi vismaya No need to worry about that As we South Indians usually given initials, where it is not used in expanded form in any major documents such as 10th marks card like that . But in passport we must expand the initials where in foreign countries wants our full name it gives clear identity about a particular person in those billions of people , so only in passport and also in PAN card we will expand our initials . so there is no worry about it . As we South Indians usually given initials, where it is not used in expanded form in any major documents such as 10th marks card like that .
But in passport we must expand the initials where in foreign countries wants our full name it gives clear identity about a particular person in those billions of people , so only in passport and also in PAN card we will expand our initials .
so there is no worry about it .

First one as
Given+middle name= Sandra suresh
Surname= Sunitha
Second option as
Given name=Sandra
Surname= Sunitha Suresh
Both options are acceptable .

Thankyou Technow for clearing my doubt. Aakhil, the order of expanding your initials doesn't matter because both the alphabets are the same(S.S). You can give your father's name first then your mother's or mother's name first then your father's in the surname section. I gave my mother's name first. Hope this helps.
