The reason why Doctor was pretending to be a fortune teller in her illusion is, In reality doctor is the one who says “K” is no more and Sam need to forget “K”. Sam was switching between Reality and illusion, so her session with doctor in the first scene was reflected in her illusion world as fortune teller. One more reality scene is toilet paper scene you can she while she hearing voice note from doctor. Red pills box is on bed which means she had a pills that y she came to reality. Once she read the notes in toilet paper she again opened the story book and went back to illusion.

Its all part of her hallucinations based on the script because all these tattoo and cactus plant continuity errors is also mentioned in the script book as a note if you see in script book scene so why can't these be a part of script too that's why you also see the last scene where she is dry is also mentioned in the script book although she will have tattoo in that scene.
