17 Shoe Design Principles for People with Internal Chronic Foot Pain

  • The condition where the foot hurts whether wearing a shoe or not. It is often associated with osteoarthritis in the big toe or ankle joint. The pain is not caused by the shoe, however you can make a shoe that relieves that pain by making it so that the joint does not have to bend.
    Bill Bird explains how you can make a shoe or boot that is very stiff but facilitates the gait cycle.
    • Creating a radius from the ankle pivot point for the heel base of the shoe
    • Creating a wheel segment from the radius of the hip for the central shoe sole
    • Creating a radius from the ankle pivot point to the tip of the toe which allows the toe to roll off
    This principle is often used for osteoarthritis but it has also been used for a prosthetic foot and for a fused ankle.
    For customers that require total rigidity in the sole carbon fiber is used.

    Superb series of videos, Bill! Multiple camera angles, professional lighting, excellent production values. I hope the viewers appreciate the effort that has been put into these videos.
    As an orthopedic shoemaker since 1976, with an interlude of 25 years as a prosthetist, I would like to add some insights on prosthetic feet and custom orthopedic shoes. Various prosthetic feet require very different fitting principles from footwear. For instance, the standard prosthetic foot of yore was the SACH (Solid Ankle Cushion Heel) foot, which depends on the compression of a soft heel wedge to replicate the motion of the ankle. There needs to be enough room for the soft foam of the heel to expand a little and clearance under the arch for the heel to depress so that the prosthetic foot functions properly. Other designs have hinges and bumpers in an articulating ankle which do not have these requirements. Some prosthetic feet can tolerate a little variation in heel height, while other designs require an exact heel height. Consultation with the prosthetist is essential in designing the lasts and shoes for amputee clients.
