Probably because Madonna felt that it did not fit Like A Prayer album. I like the song, but it could have been the odd one out the tracks. Even the title is somewhat strange. It could have surely and easily been on True Blue or as a bonus track.
Seif Eddine - Absolutely. The title comes from a popular movie from 1938 starring James Cagney. Madonna already did a tribute song to one of his films in 1986 called "White Heat" with dialogue from that film. She might have thought it was too repetitive and wanted to do something different.

This was Madonna in her prime, when she was very much a first take artist. She's of the school that "if you don't get it in the first few takes, you've lost whatever 'magic' was there in the first place". Live to Tell is her original guide vocal as is the Like A Prayer vocal (the demo vocal is the track vocal). In later years, she's been chasing the crowd and working with the "it" producers of the day, whereas up until Ray of Light, she was setting the trends and making producers famous.
