Hi, could I possibly get a copy of your scale and tests forADL ? I have had numerous head injuries over 16yrs., and been unconcious 3 times. My cognitive abilities and quality of life have been significantly impaired, but I need to prove this clinicaly here in New Zealand. Please help if you can. Shane.
