I am sorry for the late response. Think of SQL Server as a virtual computer, and think of database as a folder in your computer where you store your data or files. Think of SQL server management studio as a client connectivity tool (for example you need ssms to be able to use sql server).
hey , i was trying to update from RTM version to sp3 but always had a problem with server database engine that fails always , is there any explanation or solution
If you are asking about patching a SQL Server while it's offline, yes it's possible to stop SQL Server services before patching but that's not a required step in patching sql server

Yes, in all SQL Server service pack (SP) and cummulative update (CU) can be applied to every edition, even to free Express Edition.
