Radames is far too heavy a role for his voice - ditto Pavarotti. To some extent you can blame Karajan for persuading many singers to take on roles several sizes too big for them.
It's a fashion that i don't fully understand, almost all tenors think that singing opera is like a fight: be as strong as possible and don't mind the rest (as if a fight were like that rs). Can't they just realize that despite training their voices have healthy limits and an even smaller "land" in which they can travel with beauty and quality??? The dynamics must be varied according to the limits of the voice. In Europe even some coloraturas have sung Turandot. It depends on the size of the house and the skill of the conductor. The tenor role in Carmen was originally written for a very lyrical voice. If you follow all the dynamics in the score there are many pp's . When ever Carmen sings the orchestra goes down to nothing....of course I think you meant if they really knew their voice and it's limits they wouldn't agree to sing heavy repertoire that doesn't fit their natural voices...

A lyric or leggero tenor who tries to hit a high note in dramatic role will not be listened or will force themselves too much. Perhaps he was on bad condition, but i think that he was taking too much for his voice type, making easier that crack to happen.
B IS high and for many tenors, it's the very top of their usable range. Domingo for example. You can get by without a C, but you must have a B. And cracks can come out of nowhere, when the singer is perfectly healthy and has been singing fine in the time preceding the Crack. It very nerve wracking. As a high note approaches, a Singer can't predict what will happen. In this case he probably didn't warm up sufficiently. This happens early in the opera, and there is much heavy singer after this. He probably didn't want to "leave it in the dressing room" ( when a singer gets tired from too much vocalizing before a performance) . He wanted to have voice for the whole night, so he cut the warmup short.

They weren't pretty, but he did hit them. Some singers are prone to cracking (Caruso and Domingo) some just never do ( Bjoerling). As a former singer, I Can tell you the singer is more surprised than the audience. If it's never happened, chances are they don't sing that often. But 3 or 4 nights a week, you will have some sort of vocal situation. Cracking, or suddenly losing the voice. That's worse than a Crack. To open your mouth and no sound comes out- nightmare
