  • One of the most common questions I get asked is about retinoids - which one is the right one for me?
    Not all retinoids are created equal, and it's important to understand the differences between them in order to choose the one that offers the most benefit for your skin.
    In today’s video I dive deep into three over-the-counter retinoids; retinol, retinaldehyde and hydroxypinacolone retinoate (HPR) to help you make the best choice, especially if you have sensitive skin.

    So far I have tried retinal, granactive retinoid, retinol 0.3%, and now I am trying adapelene... With each of them, I followed ALL the tricks and tips, introduced it very gradually for months, sandwiched, removed other actives, used a lot of supportive skincare...and after some months, my skin would just get so irritated and inflamed (and it is not the typical purging of flakiness, but red inflamed bumps with small pustules). It feels like the more I go, the worse it is :( I think I might need to give up altogether 😭

    Such a great video Dr Sam 💕💕. I’ve been on your Flawless system for almost 3 years now. Not really sure how to go about increasing the Flawless Nightly, it does wonders for my skin. I’ve never had any skin irritations since day one usage. I am kindof thinking 🤔 why change what’s working. But I am not sure 🤔 if the skin adapts to this percentage, which is why we need to increase. Thanks 🙏 again for all you help.

    Adapalene (beginning with epiduo then switching to straight adapalene) has truly been a game changer for my extremely oily and acne prone skin. I had horrible acne as a teen even once having a cyst/nodule so big that my Curology derm told me to go to urgent care. The past few years my skin has started to look much more normal, i definitely still have acne but I finally feel like a NORMAL PERSON with occasional acne rather than a dirty freak with tumors all over his face. And it’s like 20 bucks per tube and a tube lasts me more than 4 months

      Hi! Are you still oily when using your adapalene? and if you don't mind me asking, what is your full regimen. I'm in my 40s and more oily than I've ever been in my life. I've gotten more oily as i've used acne products, especially retinoids. I wonder if they just clear your pores, thus letting the oil flow more easily?

      @@Juliet42110 in the morning I wash with the Zapzyt acne cleanser- super gentle and affordable, then a generous amount of a generic lightweight moisturizer, followed by a thin layer of adapalene. If the adapalene seems to be especially irritating that day I’ll finish with a thin layer of the moisturizer on top. At night I usually just wash with water because I’m lazy 😆. I’m not sure why you’d be getting more oily as you age, but if it’s any consolation oily skin is more resistant to premature aging :)

    Thanks for your clear & concise explanation of the different forms of retinoids. I’ve watched many videos on this topic over the years, but this was the best IMO. I don’t know that I knew about the packaging aspect prior to this (I know vitamin C should be in light protective packaging). Thanks again.

      I love adapalene. Easy to get it through GP in the Uk - less than 10 pounds. It transformed my skin! However still fairly drying if I am not careful with application\overuse

    I use the Granactive Retinoid, and my oh my... it reacts fast with less to no irritation on my face. Got skin purging in my first 3 days, but now, just in a week, all the effects subside. Most of my skin parts are smoother and the pores seem to shrink.

    I think my dehydrogenase enzymes are set to "turbo" because the main difference I notice on my skin between retinol, retinaldehyde, and tretinoin is speed: each gets me to the same result, but tretinoin just works faster. I can get an acute erythema the day after using tretinoin that basically looks like a sunburn, so I keep retinaldehyde on hand in case I'm going anywhere I need to look professional the day after a retinoid night. I recently learned the hard way, however, that the rate-limiting effect of retinoic acid entering the skin that using metabolic precursors like retinaldehyde produces has a slight downside: I tried a 0.1% retinaldehyde, which, in hindsight, was too strong for my skin, and got "time-released" irritation for the next 5 or 6 days. By contrast, using too strong or large of an applied quantity of tretinoin will give me irritation out to about day 3, albeit much more intensely. So even with the OTC retinoids, it's important to exercise caution: 0.06 % seems to be my skin's limit for retinaldehyde, and I discovered years ago that 0.3 % was the sweet spot for my skin with retinol 😁

    I am 65 and I am using the Ordinary 0.5 retinol every Night, no irritation. My concern : wrinkles. Should I use the Granactive retinol 2 % émulsion instead?

    Ohboy. So, I went to the dematologist. I used to have a dry skin, and a sensitive one, so I asked for non-perfumed products. I asked for a retinol or better because of my age (fine lines, big pores, uneven skin, acne and dark spots, uneven skintone). Never used a retinoid/retinol before. She suggested Avene cleanance women night smoothing cream (0.1 % retinaldehyde). I asked her about starting out and not applying it every 3 days at first, and she just said nono, use it every day (???!!??) just increase the amount bit by bit. She also said to use Avene hydra (think it's a barier repair one) as a day cream. She said cerave balm was too thick to use on face (although the pot says face and body?). Hmmmm, think I'll start out with a granactive and build my way up really. Her approach seems wild and too harsh for me. Heard people sandwiching retinols even between 2 layers of cerave. Derm also said not to use it on my neck, after I asked, but think I'll try the granactive on my neck since i have painful horonal acne there. And those won't go away for ages. Also after stopping to use cerave on my face, my eyelids are rough, raw and have started to use it again, because applying cream stung even. Rest face seems ok. She says I have oily skin, but, no? :p

    I use your night serum ‘Nightly’ and use it every day without issues. What would I have to do if I wanted to increase the potency ? Will you make more potent serums if the kind ? Or should I move to another retinoid?

      I agree! «Nightly Light» without Niacinamide. 10 % is just too much. 5 % is just at the limit for many of us. If there were Nightly without Niacinamide, it would be easier to use it together with Flawless Moisturiser for us that can handle some Niacinamide, and without for those who can’t 👌🏼

    Would you be able to cover and talk about adapalene and how it is different from the OTC retinoids talked about in this video? I have heard that adapalene is best for people with acne and not for anti aging. Is this true?

      it was designed specifically to target the pores to treat acne

    I was put on Retin-A when I was 12 for acne. I used it for about a decade and then stopped. When I hit 30 I thought I needed anti-aging help so I started on a 1% retinol serum. I have used that for the last decade +. Not sure if I should switch back to a prescription or just continue to bathe in the 1% serum.

      you get very little active product in otc retinoids so it's better economics to use the product that has decades of proven efficacy - tretinoin

    Thanks for a great video! Why is tretinoin actually on prescription in most countries? We only talk about how good it is, so one could argue that everyone should be able to enjoy it! :) Could it somehow be dangerous to use (long term)?

      it's very safe and it is available over the counter in at least 7 countries. One of the best things about it is that it forces people to use SPF daily, so it should be encouraged IMO. But like a lot of things there are people who don't do their homework and will misuse it and then blame the product for their problems.

    I have been using tretinoin but now i think it is giving me dry eyes. Should i shift to granactive retinoid. Would it be better for my dry eyes.

    I've been using adapalene (Differin) for about six months and still can't use it every day from irritation. I know it's the best thing for my skin so I use three times a week. Still the best stuff I use for the occasion break out.

      Try buffering! Apply a thin layer of moisturiser before applying your retinoid while your skin builds up tolerance to the active ingredients

      I have fair sensitive skin and use Tretinoin 0.05. What allowed me to build up tolerance was buffering as Dr Bunting suggested. Then I went very slowly, using Tretinoin one night a week for a month then two nights the second month etc. By buffering and going very slowly I was able to get my skin to adjust. Even now I take one night a week off and just use moisturizer.

      I already finished adapalene usage.. wasn’t strong enough for me , used it for 2.5 months or so until i ran out of it

    ive been at 0.025% tret for 2 years. is the goal to move up to 1%? i thought all versions of tret were comparably efficacious after a certain period of time?

      Unfortunately, we are no longer able to ship Flawless Nightly Serum, Flawless Nightly Eye Serum, Flawless Neutralising Gel and Flawless Brightly Serum to the Republic of Ireland. This due to S.I. No. 540/2003 - Medicinal Products (Prescription and Control of Supply) Regulations. Sadly, customs will not allow these specific products to be shipped. The team is continuously working to try and resume the shipment of the serums in the future, but sadly the situation is currently out of our hands. The rest of our product range can be shipped as usual!

    Wayne Goss did a video on ‘contact therapy’ using Tretinoin. I have been doing this and am up to one hour nightly. Dr. Sam, what are your thoughts on this? Thanks!

      Lisa, I’ve never heard of it, but I’m going to check it out. I use Obagi .025 every night. Anything more potent irritates my rosacea skin.

      @@TRWoods914 Wayne said that info was from a derm and that he is using that method. I’m using 0.05% Tretinoin without any irritation for about three weeks. I just can’t seem to find out any more info. Give me a shout out if you find something! 😊

      @@lisapierce525 ~ Will do. I tried .05 and it seemed fine for a while, but I had to switch back to the lower dose. My dermie said she’s surprised my skin can tolerate any strength. One thing I wanted to add (without knowing your skin concerns) is I started using Dr. Sam’s Neutralizing Gel daily in my t-zone. It’s helping with so many issues - especially on my chin. I have two backups. I’ve been using Dr. Sam’s products for a few years with much success.

      @@TRWoods914 My biggest issue is skin cancers. That’s why I started tret. I do have some hyperpigmentation and was treating that with Vit C and glycolic acid. Now I can’t use any actives or even niacinamide with the tret. My insurance pays for it, which is great, but after six months I was still peeling so that’s why I decided to try the contact therapy.

I chimed in to say I just found a video by dermatologist Dr. Dray called “Short Contact Therapy.” It’s from four weeks ago. Haven’t watched it yet - but will tonight. Maybe we’ll get more info.
