i was playing solo, this was vanilla. it happened due to an extremely rare bug in which fire escape does not spawn inside the entrance. I checked the seed later, part of the facility was cut off into the void, causing fire escape to never spawn inside.

I believe its a message that plays when something procedurally generated happens to block the other side of the door. It can be seen trying to access the fire exit inside (tree on other side) or out (shelf on other side).
