Dr. Ioannidis on Why We Don't Have Reliable Data Surrounding COVID-19

  • As the coronavirus pandemic tightens its grip across the world, is the reaction a fiasco in the making? Stanford University professor Dr John Ioannidis lifts the lid on our worrying lack of reliable data.
And, see his thoughts in writing here:
    www.statnews.c...
Head here for the full-length interview with Dr John: • Video
"We have gone into a complete panic state", says professor Dr. Ioannidis, describing the perfect storm that led Italy into crisis. The esteemed professor believes an exaggeration of the virus' mortality rate could lead the world to a fatal scenario: a lockdown lasting up to 18 months. "Then it is not just millions of lives at stake SARS-CoV-2, but billions, if we have to protract that for so long".

I've been discussing this topic with people since January. I feel like I'm bashing my head against a brick wall trying to explain it. It seems most people, even people I'd normally consider relatively smart, don't understand the basic stats. I find this very disturbing, since that's possibly reflective of the knowledge the politicians have of it.

I faced the exact same issue. The media confuses different sorts of mortality rates. The numbers particularly from John Hopkins have a sampling bias and of course anecdotal cases galore.
This isnt the first time though that as soon as I throw up my hands amd assume that the public will never learn, they prove me wrong and react with tremendous awareness. Five buck says that this is the mainstream view soon enough.

Rex Butt That is messed up if people are deleting them.
One thing that does worry me about his speech... he leans on the age factor. But we are seeing much younger people getting bad cases of this here in the USA.
I DO agree with a great deal of what he says. I particularly like the part where he talks about if we had a counter showing every death how it would sensationalize the deaths.
I do find it interesting that the politician who said that our grandparents might be willing to risk it to keep the economy and country going was brutalized. But this guy is saying the same thing.

As science became the playfield for extravert rockstar scientists and the REAL researchers got detached more and more of the policy makers, our democratic model became a really thin reality. I have been telling anyone who would listen to me for weeks that the numbers from the Diamond Princess are the only "hard" data that we have. This guy is brilliant and is a voice of sober, intelligent reasoned science amongst the panic mongers in the media and the government. As of today in the US we have about 17 million people that have lost work almost overnight. Most of those jobs will not be coming back. In addition, there are millions of illegal people in the US with no savings, living paycheck to paycheck, who will not be getting any unemployment or stimulus. This is a ticking time bomb. Anyone suggesting a lockdown of 18 months should be ignored and lose their job.

Among 381 symptomatic patients, 37 (9.7%) required intensive care, and nine (1.3%) died (8). Infections also occurred among three Japanese responders, including one nurse, one quarantine officer, and one administrative officer (9). As of March 13, among 428 U.S. passengers and crew, 107 (25.0%) had positive test results for COVID-19; 11 U.S. passengers remain hospitalized in Japan (median age = 75 years), including seven in serious condition (median age = 76 years).

The issue facing the US is not whether large numbers of people will get COVID-19. Either by accident or design, we're committed to accepting that a very substantial portion of the US population will become infected. The issue is, WHEN do all those people become infected. The timing of the infection is what distinguishes this pandemic from a normal flu season. The healthcare system can handle a bad flu season--it proved it this winter. It's clearly not designed to have large numbers of people requiring emergency intervention for a prolonged period ALL AT ONCE. What we need are policies and practices that allow a return to some form of normal life without precipitating a huge spike in infections. Downplaying the morbidity and mortality of this disease will not get us there.

Gaslighting and crazymaking are one in the same - they are both terms to describe a person that wants you to feel like you’re losing your mind. They do that by causing you to distrust your own decisions and make you think that you are wrong about almost everything.
They know how to turn you from a happy, assured individual with your own thoughts and ideas into a dependent, unsure and fragile person that can’t take any steps without checking in with them first.
Emotional abuse is insidious in nature, but crazymaking is the ultimate perception destroyer. If you can’t trust your own perceptions, you don’t even know what reality is. After some time with the crazymaker, you’ll come to trust the very person causing you to distrust yourself.

Also Northern Italy is in a higher altitude, and hearing from some doctors many of the virus causes what is similar to altitude sickness. Suppose that the car culture accelerated infection rates early around the world. People share the small spaces of car interiors. Most were likely asymptomatically infected. Logically there are stronger and weaker mutations of the virus with them all being different like people are all different. There is some risk of being infected by a stronger mutation but how great is that risk? Here in the area I live quarantining means moving in with relatives or friends rather than the government actually isolating people in pricey hotel rooms or in specially erected quarantine quarters.

He says the problem is we don't have good data, then proceeds to assuming global response is excessive, based on his preconceived notions about what good data might be. In an article of his, he's assuming this might be comparable to flu. 0,05% death rate when one in five elderly dies? Give me a break. How to bring back societies to normal after the lockdown is surely the million dollar question right now. But he's mentioning Italy where death rates multiplied because the healthcare system was overwhelmed, yet still talks of exaggeration. He's insinuating US healthcare can take it better and it took like 10 days for him to be proven tragically wrong - just read of US hospitals saying that at the current rate they will be out of ventilators in 6 days - make no mistake, if you need a ventilator and do not get it, you die. There's much to learn in the following months in order to be able to say how and when to start going back to normalcy, not just data coming through mass testing. We don't even know how the virus works and what it does to different people, antibodies etc yet. Also, any medical developments will play a huge role. Your country proceeding in a lockdown after so much "what about the economy? it might be better to just let old people die" and weeks of ignoring the fact that infections gained an exponential rate that got it the highest death count globally is not the best time to insinuate social distancing may be more destructive than the pandemic. There will be more time for that in the future, hopefully done in intellectual honesty and not in disguising the economic interests of the few as public interest.

Cripple horse, you are right. This guy is fundamentally underestimating the crisis. 1% death rate is very very high for one season. I know here in Ireland, they are flattening the curve, but from my medical friends, they tell me its pretty lethal. You have very small chance if you get put on a ventilator. It's just a Hail Mary at that stage.

    • @PinkAsAPistol
      @PinkAsAPistol 4 ปีที่แล้ว +2

@@theftauto79 Judging from my country and many others, it seems that people on ventilators roughly have a 50% survival rate, which is of course still terrible, but at least half of them survive when they would all die without it. Instead of being overly pessimistic, let's push for healthcare systems that will not run out of ventilators and force doctors to choose who lives and who dies.
But yeap, to begin with, his line of thinking leads to a frankly ridiculous conclusion: That when in doubt about the true death rates of a rapidly spreading disease, we should err in the side of not harming the economy and against trying to save as many people as possible. Apparently, we should wait for good data and have already caused thousands of people to be dying every day, as was the course that US took up to this point. Then, even conceding that some people go through this asymptomatic or without being diagnosed - so death rates must be somewhat lower - he's throwing a ton of assumptions to the public based on a best-case scenario that already seems unlikely by whatever incomplete data we do have. Like all scientists he criticizes throughout his career, he's showing clear bias towards the results he would like to be true in this case.
I'm glad of whatever countries base their coronavirus response on science. But what seems to be alarming to me is having certain political decisions taken while being masked under seemingly technocratic thinking. So let's say Ioannidis' gut was right about deaths being closer to his best case scenario (of this virus being like a flu?). Who decides when "the treatment is worse than the cure"? Will we see Ioannidis demanding scientific data regarding how many people would die due to economic difficulties - as he's seemingly concerned about - in order to pitch them against coronavirus death rates and be able to make "evidence-based decisions" about the necessity to lift a lockdown? For some strange reason - as strange it is for an epidemiologist to be presenting the argument for the economy instead of public health - I'm sure that would never happen. Who decides what death rate is acceptable then? And what's this "economy" we are talking about? Does anybody honestly believe that a "workers first" approach would lead to the same decisions as a "corporations first" approach on deciding when the economic effects are worse than letting people get sick? Would we let such decisions to technocrats? What economic policies are taken for granted here? Maybe taxing the rich and providing universal income instead lowers the death rates that make a lockdown seem like the sensible thing? So I'm sorry to say I see a clear Trumpian agenda beneath what Ioannidis seems to be saying. And it's not the plea for more scientific data that is concerning, but what preconceived political decisions they are supposed to support once we have it.

I do get the bottom line of what he is saying but the thing is that CoVid 19 absolutely overwhelmed the healthcare systems of so many different places apart from Italy, we have a high percentage of people who were not considered to be in the risk group needing hospitalisation, therefore social distancing and some sort of short term lockdown is totally necessary.
