    Hi Chris, I'm a 22 year old male based in the UK. I've been experiencing low back pain for just under three years now. Began as a mild discomfort at university when sitting and has now escalated to a level where prolonged sitting (particularly in trains, planes and cars) is very painful and causes flare ups which can leave me bedridden for 1-2 days. MRI says non-compressive disc bulge of the L4-L5 disc with a small high intensity zone and facet hypertrophy. My pain is confined solely to my lower back with no radicular leg pain. NHS (National Health Service) have told me to avoid invasive surgery and have said that there's nothing more they can do and to simply keep active and hope for the best. However, I've taken my scan to a private clinic in London and they've told me that I do indeed have an annular tear of the L4-L5 disc (which from what I've read on your site matches my symptoms). I had steroid injections into my facets back in October and was prescribed three months of hard physio, although this has had little real impact. In my research I've found "the regenerative clinic" in London offering Lipogem treatments for a host of orthopaedic conditions including discs and have also found Algocells which I believe are a sister company of Regenexx in the USA? I've read your blogs on the scepticism surrounding Lipogems and as far as I can tell, your clinic is the most experienced on the planet when it comes to stem cell treatments. With that in mind I am wondering if a) this is something that it is treatable by stem cells and b) if I would be able to have the procedure done in the UK via Algocells to the same standards as your Colorado clinic?

      Surgery is not always successful and is irreversible. Stop doing anything that induces pain and find other painless solutions. Never lift anything heavy or put any strain on your body. Don't run or jump. Learn to walk without missing steps or slipping. Learn not to twist or bend too far. Imagine you have an egg in your back that you must not crack. Buy a water bed and fill it until your spine lies closest to its standing alignment. Start cycling 50/100 miles every week. Eat a balanced diet and do not exceed average of 2500 calories. In short do everything you can not to make things worse, but to assist your spine for the best possible recovery.

      Iazzaboyce would this routine you've mentioned make me do the stuff I love such as soccer and raquetball after it's healed? Perhaps in one or two years?

    1:20 If the disc is torn, won't stem cells leak?
    Also, I don't quite understand why epidural PRP would help reduce a bulge. I hope it works.

    How about SCHMORLS NODE HORIZONTAL HERNIATION ..HOW CAN STEM CELL HEAL BROKEN ENDPLATES..ANd MAKE THE NUCLEOUS GO BACK INSIDE AND CLOSE THE ENDPLATE????..i have endplate scmorls nodes in CC5C6 AND C5 C7 ALOT OF PAIN MOST DOCTORS ARE IN DENIAL OF

    Can stem cells work with spinal stenosis? How do you shrink the areas where the nerves are pinched?

      This requires a precise injection into the bulging ligamentum flavum and into the ligaments taht stabilize the spine.

      @@billylim7276 hi billy i had radiofrequency surgery on 1 September from l3 down to s1 and for the whole month i felt so great but starting October the pain came back and to be honest I've no idea what happened I've promised to be relieve for at least 6 months to a year pain free but not the case so now im lost i've called my doctor and told him the pain came back little by little so he told me to came back but im not optimistic my issue is Facet joint syndrome

      @@johnnyrodriguez276 Johnny hi unfortunately médecin sucks in back problems this is what I've figured out going to multiple doctors but the last thing i did is radiofrequency and i can swear to you that im in relief pain about 90% of my pain goes away but sure it's temporarily so honestly i don't mind to do it again after the nerve grows up again with the radiofrequency i don't do sever activities so if in the long run radiofrequency will be bad the last option is to replace the disk or make a fusion if there is no new technique is available I've heard about a new procedure calledthe intracept procedure look at it if it's suitable for you and tell me what going with you don't have suicide thoughts i had them for a while but don't worry temporarily solution still exists and start accepting what you have believe me after all a part of this disease is mental stay safe

    Hi Dr. Centeno. I have cervical spondylosis/osteoarthritis of the neck for about three years now. How can stem cells or any kind of regenerative medicine for that matter help me to avoid surgery? I do physical therapy. Have had Epidurals, Medial branch blocks and radiofequency ablations. All of these treatments only temporarily help. Have there been people who have actually succeeded with stemcell therapies/regenerative medicine for neck arthritis? Do you think progress can be made and that the FDA can approve of most, if not all stemcell/regenerative medicine for chronic spine conditions to heal sufferers? Please reply and thank you.

      Read about there phase 2 results on this indication. Research how you can get involved. Maybe you can qualify. It’s going on as we speak. Phase 2 was a Yuuge success. Read outs for phase 3 will be 2-3 quarter of 2020. Looks as though it’s a huge game changer

    I have a herniated disc bulge c6-c7 and it's caused a syrinx to form, so I end up with pain from both the inpringed nurve and the spinal cyst.
    I would rather avoid a shunt or spinal fusion, would the platelet treatment be something that might help?

    I just got one more question Dr Centeno, so I understand that a torn disc can cause back pain, in herniated disc, what aspect of it causes back pain, the torn disc fibers? Loose fragments? Pressure on a nerve, its confusing to have a herniated disc and lots of aching pain but when other people talk about it they just refer to leg pain, thank you for your replies doctor

      Dr. Shah will be able to correctly diagnose the problem. You might actually have a painful and torn disc which cases “disogenic pain”. The outer covering of the disc can be torn due to injury and painful nerves can grow into
      the area, leading to low back pain, usually with sitting or activity. In addition, the tear in the disc may lead to noxious chemicals escaping and irritating the spinal nerve, which can lead to buttocks, or, leg pain. Not everyone with a spinal nerve affected by a low back disc has leg pain. Diagnosis is much more complicated than that, and MRI's, and certainly MRI reports, can be misleading.

    Hi Dr Centeno, I have a cpuple of questions, so I know that annular tears can cause lots of axial low back pain, but what would be causing axial back pain in a herniated disc? I know most people mainly complain about leg pain but I have a confirmed herniated disc looled at by many doctors on my MRI, what part of a herniated disc is likely to be causing the axial low back pain which gets worse with sitting on anything that causes pressure, and can that be helped with stem cells? Also I have severely tight hamstrings, can that be from the disc pressing on a nerve? Because when I think of nerve compression I think numbness and tingly sensations not tight hamstrings, thank you

      Yes, in fact chronically tight hamstrings are often a sign of a smoldering low back problem. The different disc problems can be a bit confusing. The difference between an annular tear and a herniated disc is that is that in an annular tear, the disc is torn, but it's contents stay put. A herniated disc just means that the outer covering of the disc ( the annulus) is completely torn or damaged and the inner gel squirts out. This can put pressure on a spinal nerve which can create symptoms in the area that the nerve controls, or pain, or, the numbness and tingling you're more familair with. Each case is different and of course we'd need to see what's actually going on by reading your MRI oursrselves and do a thorough exam, but with a Herniated disc it’s usually best to stick with a technology that doesn’t inject the disc, but instead instills isolated growth factors around the irritated nerve called platelet lysate, which is less invasive than a disc injection. The focus is to ramp up your body’s own natural repair mechanisms, which can get rid of the herniated disc.     I am having really bad chronic back pain for almost 9 months now. I am on a medical leave from work. L4 L5 S1 facet ostheoarthritis bilaterally and bulging discs. They are both mild according to MRI but I am in so much pain. Im 23. Could this help?

      I am so sorry Mihai, I know first hand how debilitating this pain is; the suffering is unimaginable. I have had numerous epidurals for degenerative disc but just shooting some pain meds in it was hit or miss and didn't last long. I heard about stem cell epidural injection from a father that had a 23 yr old daughter unable to walk from the pain. He took her to Dr. Lee (Regenesis), now she is in the Air Force. Dr. Lee extracted bone marrow from my hip. He used a procedure to enhance my stem cells. He says this procedure is to increase the count; it takes 2 hours for the best results. At 4 million cells, he combined it with a gel and inserted it into my spine via epidural. The stabbing pain left within 8 hours; the best sleep I've had in years. After 2 wks of rest, he ordered physical therapy exercises for my back, hips, and knees. McKenzie wrote the book on exercises that are used by physical therapists today. Look it up. These exercises may help you right away. It was almost too simple for me to believe. The insurance companies here only allow the doctors to treat one thing at a time. So if I step on a rusty nail just pulling it out is not enough. Its all connected and you have to treat it that way. I am 90% free from back pain now & working my way to 100%. I recommend prayer and stem cell epidural injection.

      What did the stem cells do for you? they don't increase disc height from what I read above... I have degenerative disc - severe narrowing L5-S1 I am thinking disc replacement but of course if there was something else... I don't just need pain relief though. I need disc height as right now it's bone on bone.

      I have 3 2.5 mm herniated discs and mild canal stenosis according to the MRI also and that pain does not feel mild I had to quit my warehouse job and I still get nerve pain down my knee if I walk alot or stay standing at work and right now is my rest day no nerve pain down knee but pain on lower back can someone please help me on what can get me going to my normal life im just 29 yrs old God bless I don't wish this pain on anybody 😢

    I have had facet pain for about 5 years. I have done 2 RF ablations and all kinds of treatments. Can Stem Cells heal facet joint pain for good?

      Mike Harrington Mike, a lot of that will depend on the level of damage to the joints and now the level of damage to the stabilizing muscle done by the radiofrequency procedures. Suffice it to say that it's likely we can make the facet feel better without destroying it the way radio frequency does.

      I researched and supposedly there is people who have died from this procedure not just that but can cause complications like Blood transfusions, infections, bacteria E coli
