Jokes aside, it's based on an old novel in which keeping a person alive in this state was revolutionary medicine. The question it asks, at its core, is if you could keep someone alive, should you? Is that sort of existance, filled with literally nothing but pain, worth living?
The movie was also the inspiration behind Metallica's hit single One, and they used several clips of it in the music video for that song, after having literally bought all the rights to it to skip through the legal process. The Golden Rule!
And eventually he could no longer tell whether he was awake or dreaming. In such a situation, how can one tell what's real and what isn't? It would mess with you.
The Iron Maiden should be in the top 1, having immortality and getting revived from drowning over and over again for more than a hundred years is awful.
Prometheus had it terrible. A vulture would rat his liver and it would grow back and on and on it goes.

@Kieran WASTIE I was talking about Blackheart's case. The Penance Stare didn't work the first time since he didn't have a soul, but once he got a hold of the Contract with all those souls attached to it...the Stare worked. VERY effectively.

In the film “Saw” (I don’t know which one) there’s a device called “the brazen bull”. A real life torture and death device where a human is tied up and put (through the side) in a hollow bronze bull that has holes where the nostrils are (because the sound of the victim screaming goes through tubes and turned into the sound of a roaring bull) and then a fire is set under the bull.
The real device is shaped better.
One that i can think of is "The Human Centipede", imagine being the last person in line you have to eat excrement of 100 peaple for breakfast, lunch, dinner and snack.

Surprised that Johnny Got His Gun wasn't on this list. The main character spends the whole movie in a hospital bed after a bomb took his arms, legs and face.

Why is Drag Me To Hell not on here? Screaming in agonizing excruciating burning pain for eternity trumps everything on this list.

Dang! You missed the fate of Henri Delambre in CURSE OF THE FLY. Being teleported from Point A to Point B, when in fact Point B has just been smashed to pieces, leaving Henri beside himself, so to speak, is horrific.

That's leaving out the iron maiden. Iron maidens are lined with spikes and were already made to be a worse fate than death, or at least worse than a typical death. Possibly worse than being crucified, but in the same ballgame in experience.
I saw The Old Guard and got to admit, that’s a terrifying death to have over and over.

There's two scenes from a 2005 movie called The Brothers Grimm in which one scene involves a girl getting her face stolen by a mud creature then gets absorbed and another scene where another girl gets eaten alive by a horse that has spider webs coming through its mouth, but both of them comes back alive in the end of the movie

The cruelest fate I've ever seen is the one from Black Mirror's Christmas special episode where the guy gets locked up in a room with the same song playing over and over again for 1000 years/minute.

Actually, according to a website that came out around Toy Story 2's time, Stinky Pete actually grown to like Amy because he was finally being played with, after becoming so bitter at toys (especially space toys) for spending a whole life on a dime store shelf

I’m sorry, but the Sarlacc from the Special Editions just reminds me of “Shut up Meg.”
For the mention of pirates of the Caribbean, they can feel pain, is shown in several battles when they are cut and stabbed as they groan, they only cannot feel pleasure. Even Barbosa says on that scene that he's been parched of thirst and starving to death for a long time.

"Freaks" (1932) - the revenge upon two antagonists was really disturbing and the movie was banned in some countries. For instance, in the UK until 1963.

Trapped in Limbo - Inception. Imagine Waking Up After Living Your Entire Life...
Darth Vader's was pretty horrible. Dude lost all his limbs, was burned to a crisp, and had to deal with the guilt of believing he killed his pregnant wife.
Given the piece of shit had tried to murder her, betrayed the jedi murdered children in cold blood, that was too kind of a fate for him. Especially given the fact that he was given mechanical replacements and able to live to do more evil.
Not a movie but theres this doctor who episode I vaguely remember where this guy falls down for all of eternity. To me the isolation without end scares the hell out of me. Adding eternity to the easiest punishment turns it so much worse.
