"Patients are going to do what they want." Probably a good rational expectation. But they can also follow instructions. You talk about using novel atypicals to try and avoid clozapine because of the prescriber burden, but what about MAOIs? I get the sense even a lot of psychs expect patients to say "thank you doc. Now, irrespective of what you say, I'm going to go take my selegiline, and then eat from between five on up to twelve wheels of gouda immediately." Especially for atypical depression where the third SSRI probably isn't going to do anything more than the first two trialed.
