The Dutch woman actually came in to have an abortion, and she's willing to pay him _way too handsomely._ But the Doctor doesn't want the money (because now he's absolutely in love with her) and creepily suggest she come by his office after hours. The Dutch woman, rightfully, refuses and storms out in disgust and frustration. His new obsession with the woman causes him to *stalk her to her house* and that makes her even _more_ distant and afraid because she's trying to keep her pregnancy a secret. So instead she goes to a local healer for one but process is botched and she dies. So, as the true shining knight in armor that he is, the Doctor makes it his mission to protect the woman's secret and prevent anyone from knowing the actual cause of her death. Like he's borderline OBSESSED with this mission. So he forges death certificates, follows her husband on the Oceania (but avoids him the whole time). Doctor finds out about an accident in cargo so now he knows where her body is being held. So he jumped onto the coffin and drug them both to sea. Literally no clue where this untouched on virus or serial killers or puzzles n' shit come from lol
That explains alot, what i got out of the first meeting was that he wanted sexual favours instead of 12.000 pounds, and thats why he feels guilty, also the location was not clear at all other than it was not in Europe i guess.
The ending is strange because it's supposed to be an adaptation of the book, but the book's ending was different and maybe made more sense. In that ending the doctor threw himself onto the coffin that was fastened to ropes, thereby dragging both the coffin and him down to the bottom of the sea. Neither could the person running amok be saved nor could the coffin be recovered. So him drowning with coffin within it maybe made more sense there.
