Can you add pain as a nursing need on your careplan if the patient complained of pain?

If you write the 'today (the date) and weekly' as the re evaluation date would you be marked wrong?

Yes , it will be wrong because you are not going to leave a week for a patient in a hospital scenario without assessing their nursing need again.

I am from the US and have noticed that there are many spelling differences between the UK and USA. Recognize vs recongise...Are responses graded for spelling errors also?

Mam, upto 5 liter is really adviceable to drink. So how this person will eat?

Its to aim for between 2-5 litres as tolerated by the patient .
