That ghoul just spawned by maiden passive, and there was no enemy nearby so the ghoul go to follow yorick that was in midlane. (Sorry im not english speaker)

Xayah is pretty spaghetti in general, you have to die to fix her Q speed

It's a bug, sometimes for whatever reason xayah's feather lodges itself inside of a champion's hitbox, so when they die and take it to fountain her e is global
