My question is how do we know we're in ketosis at any given moment. According to Professor Kay, when we eat a high protein meal, we most likely come out of ketosis for a certain period of each day. Maybe if you never eat enough protein to bump out of ketosis is where potential problems MAY materialize.
I'm a metastatic cancer patient and have been on Ketogenic diet since October 2019 1 yr after my cancer diagnosis. I also do Intermittant fasting though it's difficult to do while on treatments that force you to eat something to take the meds with. However I try to fast during my recovery periods in between the cycles. Keto has reversed my type 2 diabetes, I'm no longer on high blood pressure meds and no longer on statins either. I've also brought my weight down from 252lbs down to 180lbs and still need 20-40 more lbs to go to get to my ideal weight for my age and proper body mass. My cancer has been stable for the past year and I believe it's due to my body in more of a healthy metabolic state. My goal is to starve the cancer as it can't use ketones to feed from. The best I can hope for is to put the cancer into some form of remission. I also have totally abstained from all fructose nor do I ingest any form of extra glucose except what my own body produces. Even that is kept to a strict minimum. So I've reached and past my 5 yr cancer survival period and still going strong!

Eating a ketogenic diet for 7 years isnt the same as being in nutritional ketosis for 7 years. To know that, you would have had to monitor your ketones 24/7 for 7 years. I don't mean to be a dic#, but the terminology confusion is what opens a window for criticism from the high carb mainstream "gurus". I'm not really interested in engaging those folks. What I know is I've consumed fewer than 30 grams of carbs (typically less than 20) every day for 7 years. If someone wants to dismiss that as insignificant for whatever reason, fine; they can do that. Whatever the details of my metabolic state and physiology, I'm content with the practical reality of being able to be as active as I want, engaging many physically intense activities while also working manual labor jobs, including a job that had me working 12 hour shifts 7 days a week. I also happened to earn a PhD while eating high fat low carb. So it works, whatever precisely is happening.
I adopted a low carbohydrate / ketogenic diet 7 years ago for insulin resistance and T2D resistant to metformin. I reversed my T2D in a few weeks and went on to lose 30 kg with no efforts at all, adopting a 16/8 or omad life style. My fatty liver has regressed and triglycerides are very low matched with a small rise in cholesterol bound fats. During the weight loss phase I would check my ketone levels (0.2 - 2.5) together with my blood sugar but I rarely do so now. With the approbation of my oncologists I continued my diet through an 8 month chemotherapy for a generalised lymphoma and the exceptional tolerance and response to this treatment was attributed by them to the low carbohydrate diet. I occasionally introduce carbs and as a result experience hunger pangs and low energy that indicates that I am still insulin resistant. Insulin resistance and carbohydrate intake is a slippery slope on which I no longer venture. A retired physician, at 79 I am back to university to study languages and I definitely believe that for me this diet improves cognition. I have recently had a lithotricy for kidney stones that were present before my diet but have increased in size and the question is whether calcium oxalate stones develop under this diet. The challenge is isolation and incredulity with my peers, many of whom have adopted this diet.

People wanting to be doctors study "medicine" not holistic health treatment... Information given by my doctors was so obviously nonsensical so I did my own search and found Keto. I used to rattle with all the pills they prescribed and di noting to solve any of my conditions yet a keto diet and complete ban of sugar or carbs has seen so many conditions clear up and cannot remember a time when I felt so well. I am reduced to one blood pressure pill and may well be pill free at my next doctor visit. my personal experiment concludes that we have been lied to for decades to our health detriment and keto is great.
The other side of this coin is the 'holistic' 'functional medicine physician ' I went to who wanted to feed me hundreds of dollars of supplements forever; far more pills than my doc ever suggested. I gave it a 6-month good try with no discernible benefit.
It's amazing that so much effort goes into trying to disprove low-carb diet benefits! Is not the opposite - a high-carb/sugar diet - the very pathway, over time, to Type 2 diabetes? Do some reading on mental decline, often termed Type 3 diabetes, as well as other metabolic ailments and then consider whether low-carb or high-carb is the better route for long-term health. A Keto Diet does not ban ANY foodsort. Choose to your heart's content, as long as daily carbs are kept within the accepted boundaries of moderation. Thanks for the info. Just bought a KT mojo meter and did a 1 day fast to get into Ketosis for weight loss. I felt great at work without eating, better than normal actually and I'm only at .6 today but my Glucose looks great at 100 and weight is down about 1 lb. I only drink water now (and black coffee) no artificial anything (or "natural flavors"...)
Whole foods combined with daily fasts of 14+ hours is key. "Natural flavors" always makes me laugh.. but n9t in a happy way. Be kind to yourself and eat well (healthy and enough) when you do eat.
this is a great point, Dr. scher, that any diet can induce disordered eating. this is not keto diet specific. in fact, what is disordered eating? any healthy dietary pattern deviating from an ultraprocessed food diet?? I have done the fodmap diet various times which is also accused of being dangerous long term and induce disordered eating. I opinion that both keto diet and fodmap diet can rectify disordered eating as both diets provide a clear eating plan for health minded individuals, containing nutritious foods that contribute to keeping people healthy.
I was keto for about one year but was always dehydrated and I did everything I could. After all I decided to add carbs back in form of fruits, honey, some vegetables, raw milk and sweet potatoes. Now its a bit better but still I'm dehydrated. Really, don't know what to do now.. My hormones are imbalanced for sure but I really don't know how to fix this issue..
My husband is also a hyperresponder! He's got the same problem with LDL. Only doing the carb up to appease the GP and bloodwork to look good so GP doesn't freak out. Wants him to go on high dose statins. No way José!!.
I first adopted a ketogenic diet about 15 years ago, dropping net carbs to under 30 and losing nearly 50 lbs, which brought me from Class 1 obese to normal for my height. I know now that 30-50 grams of carbs will hold my weight steady. Over that range for more than a few days and I'll begin weight gain. Under it and I'll lose. No metabolic issues and great panels. In contrast, the Dietary Guidelines for Americans recommend between 225 and 325 grams of carbs a day....TEN TIMES what I consume. No wonder nearly 40% of American adults aged 20 and over are obese. 71.6% of adults aged 20 and over are overweight, including obesity. Healthy carbs, my ass! Way to sell us out to Big Ag and Big Fast Food, Mayo Clinic and FDA!
i was keto for over 15 years. recently added some carbs back (ca. 75-100g) to recover the tanked libido and to restore carb tolerance. previously on keto, when i added some fruit or rice, my physical performance immediately crashed for a few hours (-> a consequence of insulin suppression on keto). also SHBG is higher in keto, which means lower free Testosterone. since adding some cabs back, my strength in the gym increased between 30-50% depending on the exercise!
Can I eat real human specific food? Is it safe to naturally burn the fat on my organs that drives all-cause mortality? We are not aware of any studies that have specifically looked into this. But since ketosis tends to help the brain function "better" in many ways, its plausible that it could help.

But what about potassium and other micronutrients not mentioned here? I am a huge proponent of Ketogenic but it must be accompanied by supplementation. My brain feels great but I lose my hair en masse and am deficienct in several vitamins and minerals when on it
Thank you for your comment. Most people eating an omnivore keto diet get all the nutrients they need. For example, meat and avocados are both very good sources of potassium. And since leafy greens, above ground veggies, and nuts are compatible with a keto diet, many can achieve all the nutrients they need. Of course, there is some individual variation, which it sounds like you experienced. Sometimes working with an experienced dietician can be helpful to see if there are any changes someone can make or help tailor supplementation as needed.
but there is a physiologic need for periodic insulin spikes.. look it up. best to stay in keto most of the time, but add back some carbs every few days to keep insulin signaling going long term.

If you’re quite skinny (no body fat) take er easy… keep some gut biome helpful carbs in there, to level out high cortisol = sleep better. (I eat on keto side, but can’t sleep on very high fat/carni/very low to no carbs) Besides Vit D & Mag etc, NATTO is my idea of ‘must-do’ supplements. No grains, processed, nightshades etc. I sneak in 1 tbsp org cream w/morning coffee, but would be Much better off if I could wean from both . . . (yes, coffee raises cortisol)
@@Man-u-flex It works for some, for me trypto only works for sleep when tucked into a large Turkey meal, w/plenty of dark meat, unfortunately. Any more tips on dosage etc very welcome. But pretty sure tryptophan does not work directly on high cortisol, simply raises melatonin release. A start, but cortisol is not easily subdued.

Look up Dr Ken Berry and Dr Eric Berg on keto and thyroid. Lots of people have success with keto for thyroid, but read up first so you know how to do it healthily. Also, make sure you getting your thyroid numbers tested regularly so that any adjustments to your medication can be made. I have hashimotos and have only been keto for a month or so and my antibodies are coming down for the first time ever (I’ve had hashis for decades).
Hypothyroidism is not a strict contraindication to being in ketosis. Although, as with any medical condition, closely working with one's provider is important. @metabolicmind keto helps inflammation, and omega 6 causes it, and yet people write online the endless benefits. Many complains are inflammatory conditions that they don"t have anymore after following the diet. Yet it's high in omega 6, which is inflammatory. It's confusing.
Are you saying a keto diet is high in Omega 6? I just want to make sure I understand, But if that is what you are saying, I would have to disagree. Most Omega 6s in the industrialized world come from seed oils and processed/packaged foods, both of which most people in ketosis avoid. So it tends to be a low Omega 6 diet.
You forgot the best source of vitamins and nutriens in ketogenic diet and that´s eggs. Ketosis doesn´t stop after 6 months unless you start to eat carbs again. I´ve been on keto for 15 months now and only times i have dropped off ketosis were at the begining when i ate too much carbs couple times. Even that i had fat adabtation, i couldn´t eat much of carbs without falling off ketosis. Now after several months of keto i can eat more carbs but i just don´t. I still try to limit my carbs for 20 grams per day. But if i eat more that means that i´ve eaten alot of vegetables and some of those carbs are fiber that does not absorb in the body.

I was shocked because we have so much data on the Adventist Chistians who have death records back to the early 1900s and are currently the longest living culture on Earth. I found the quote on the WHO site and it is over a document attempting to answer this question, and it does by recommending plant based diets. All health organisations I can find have no doubt that plant based diets have no rivals regarding health statistics and research.

Dr. Scher is a physician and has to abide by professional ethics of the medical profession. He is compelled to abide by those rules, which no doubt include something to the effect that he can’t just give medical advice to people wily-nily if they aren’t his patients and if he hasn’t gone through the long list of medical issues a particular person has and knows what drugs and supplements they are taking and what interactions there could be. There are plenty of people who also need to be on other kinds of diets for their health issues - like an anti-histamine diet, or a diet rich in some nutrient that they lack, or a diet that avoids all kinds of allergens, or a diet that doesn’t interact with some life-saving medication.

The whole point of becoming fat adapted is that, having low carbohydrate stores DOESNT cause symptoms because your brain/body are still fully fuelled with fat and ketones. I can understand caution if you take some meds but if you are otherwise well, I can't see why you'd faint. I'm an MD and have treated hundreds with low carb and keto diets, many because their endocrinologists don't understand the metabolism of ketones at all.

I’d guess the long term effects of Keto diet depend largely on what types of food an individual replaces their carb calories with. If you eat a ton of red meat everyday, that could really be bad for your health.
Could you please provide any published interventional studies, not epidemiological "turds," that demonstrate a direct causal relationship between meat consumption and negative health outcomes? Given the scarcity of such data, we should not dismiss the findings from meta-analyses of observational studies. That's why I said, "it could", not "it definitely would."

Same here, 6 years on keto, pain went away, got off most meds. Never going back.

2 yrs in keto. I did have an adjustment period where I had to increase some micronutrients. But the longer I do KETO, the better I feel.

Thanks for the info. Just bought a KT mojo meter and did a 1 day fast to get into Ketosis for weight loss. I felt great at work without eating, better than normal actually and I'm only at .6 today but my Glucose looks great at 100 and weight is down about 1 lb. I only drink water now (and black coffee) no artificial anything (or "natural flavors"...)
Whole foods combined with daily fasts of 14+ hours is key. "Natural flavors" always makes me laugh.. but n9t in a happy way.
Be kind to yourself and eat well (healthy and enough) when you do eat.

this is a great point, Dr. scher, that any diet can induce disordered eating. this is not keto diet specific. in fact, what is disordered eating? any healthy dietary pattern deviating from an ultraprocessed food diet?? I have done the fodmap diet various times which is also accused of being dangerous long term and induce disordered eating. I opinion that both keto diet and fodmap diet can rectify disordered eating as both diets provide a clear eating plan for health minded individuals, containing nutritious foods that contribute to keeping people healthy.
I was keto for about one year but was always dehydrated and I did everything I could. After all I decided to add carbs back in form of fruits, honey, some vegetables, raw milk and sweet potatoes. Now its a bit better but still I'm dehydrated. Really, don't know what to do now.. My hormones are imbalanced for sure but I really don't know how to fix this issue..
Add electrolytes like Potassium, magnesium and sodium will hydrate your body.

Way to sell us out to Big Ag and Big Fast Food, Mayo Clinic and FDA!
i was keto for over 15 years. recently added some carbs back (ca. 75-100g) to recover the tanked libido and to restore carb tolerance. previously on keto, when i added some fruit or rice, my physical performance immediately crashed for a few hours (-> a consequence of insulin suppression on keto). also SHBG is higher in keto, which means lower free Testosterone. since adding some cabs back, my strength in the gym increased between 30-50% depending on the exercise!

We are not aware of any studies that have specifically looked into this. But since ketosis tends to help the brain function "better" in many ways, its plausible that it could help.

But what about potassium and other micronutrients not mentioned here? I am a huge proponent of Ketogenic but it must be accompanied by supplementation. My brain feels great but I lose my hair en masse and am deficienct in several vitamins and minerals when on it
Thank you for your comment. Most people eating an omnivore keto diet get all the nutrients they need. For example, meat and avocados are both very good sources of potassium. And since leafy greens, above ground veggies, and nuts are compatible with a keto diet, many can achieve all the nutrients they need. Of course, there is some individual variation, which it sounds like you experienced. Sometimes working with an experienced dietician can be helpful to see if there are any changes someone can make or help tailor supplementation as needed.

In about two months I plan on increasing carbs to 30-60 grams per day. This amount will allow for that small bowl of ice cream or slice of pizza.
If you’re quite skinny (no body fat) take er easy… keep some gut biome helpful carbs in there, to level out high cortisol = sleep better.

Look up Dr Ken Berry and Dr Eric Berg on keto and thyroid. Lots of people have success with keto for thyroid, but read up first so you know how to do it healthily. Also, make sure you getting your thyroid numbers tested regularly so that any adjustments to your medication can be made. I have hashimotos and have only been keto for a month or so and my antibodies are coming down for the first time ever (I’ve had hashis for decades).
Hypothyroidism is not a strict contraindication to being in ketosis. Although, as with any medical condition, closely working with one's provider is important. Are you saying a keto diet is high in Omega 6? I just want to make sure I understand, But if that is what you are saying, I would have to disagree. Most Omega 6s in the industrialized world come from seed oils and processed/packaged foods, both of which most people in ketosis avoid. So it tends to be a low Omega 6 diet.
Hey, doc. You do not mention any carnivore sources of vitamin E. The fat in meat, poultry, fish, and dairy are all good sources of vitamin E.

Ketosis doesn´t stop after 6 months unless you start to eat carbs again. I´ve been on keto for 15 months now and only times i have dropped off ketosis were at the begining when i ate too much carbs couple times. Even that i had fat adabtation, i couldn´t eat much of carbs without falling off ketosis. Now after several months of keto i can eat more carbs but i just don´t. I still try to limit my carbs for 20 grams per day. But if i eat more that means that i´ve eaten alot of vegetables and some of those carbs are fiber that does not absorb in the body.

Dr. Scher is a physician and has to abide by professional ethics of the medical profession. He is compelled to abide by those rules, which no doubt include something to the effect that he can’t just give medical advice to people wily-nily if they aren’t his patients and if he hasn’t gone through the long list of medical issues a particular person has and knows what drugs and supplements they are taking and what interactions there could be. There are plenty of people who also need to be on other kinds of diets for their health issues - like an anti-histamine diet, or a diet rich in some nutrient that they lack, or a diet that avoids all kinds of allergens, or a diet that doesn’t interact with some life-saving medication.

Many of the kids who are on keto for epilepsy only need to take it for a few years, and they can get sustained seizure suppression. That is not always the case for metabolic or mental health. But we are curious where the evidence is stating long term keto is harmful. We haven't seen it.

The whole point of becoming fat adapted is that, having low carbohydrate stores DOESNT cause symptoms because your brain/body are still fully fuelled with fat and ketones. I can understand caution if you take some meds but if you are otherwise well, I can't see why you'd faint. I'm an MD and have treated hundreds with low carb and keto diets, many because their endocrinologists don't understand the metabolism of ketones at all.

Inuit have zero heart disease which is the #1 killer for Americans for the past several decades. Traditional Inuit do not have ‘short lived lives’, and when they do die, there’s no evidence that ketosis has anything to do with it. Yet, your comment contradicts itself - if Inuit have a special gene so that they can eat low carb without getting into ketosis, then they are not on a ketogenic diet (according to your speculation), and if they are not on a ketogenic diet, then how is it “Mother Nature” has ‘spoken’ about them with respect to ketosis? If what you say is true, then Inuit diet has no relevance to the topic of the video - ketogenic diet’s long-term safety - b/c the Inuit, according to you, don’t eat a ketogenic diet. I’d guess the long term effects of Keto diet depend largely on what types of food an individual replaces their carb calories with. If you eat a ton of red meat everyday, that could really be bad for your health.

Given the scarcity of such data, we should not dismiss the findings from meta-analyses of observational studies. That's why I said, "it could", not "it definitely would."
