- On June 6, 1992, Copa Airlines Flight 201, a Boeing 737-200, crashed in the dense jungle of the Darien Gap while en route from Panama City to Cali, Colombia. The aircraft broke apart mid-flight and plunged into the rainforest, killing all 47 people on board.
Investigators determined the cause to be a malfunction in the aircraft's navigation instruments, leading to spatial disorientation. The autopilot, receiving erroneous data, directed the plane into a steep dive. This tragedy marked the first major accident in Copa Airlines' history and prompted significant upgrades to the airline's safety procedures and aircraft maintenance practices.
