We are staying in Medavakkam now and I have proof of Rental Agreement and Gas connection only. Whether I can proceed with that in Medavakkam Urban Health Centre. Pls help...

This needs to be sorted out with your UHN. Usually the husband's aadhar card is asked for. Try to do it in your native if it's rejected.
Contact the PHC/UHC block officer they'll sort out
