This is an electromechanical siren used as an elevator alarm, a siren that uses a rotating chopper with air inside of a case with chopper slots which produces sound. Pretty rare find if you ask me.
I guess this "I fart" button is a warning button, so if you fart inside the elevator you can push the button and the next passenger know he has to wait a few minutes or he has to smell your ....... Something like an ABC-Alarm :D

*DISCLAIMER:* This video was filmed in Denmark, so for clarification, the "i fart" is supposed to mean "on its way"; in this case, the elevator is on its way.
So in this elevator there is no phone? Just the bell?
There's no phone but there is an invisible 2-way intercom that will forward you to an operator when you hold the alarm.button pressed long enough. It can vary between 5 and 30 seconds. But some alarms dial immediately when the alarm button is pressed
