It's a little poetic, but the song is broadly about someone who feels very alone and is missing either someone (or people) from their past or maybe the person that they once were when they were younger. That's my interpretation, anyway. Hope that helps :) Quick edit: It probably does have to do with the person in the song being a drug addict as well with the "A little narcotic warm on me; What would I do without the weight of you" line at the beginning. But you could interpret the song as a whole as either the person in the song giving up on their life and their friends and finding some comfort in drug use and wishing they could be the person they once were. Or realizing that their drug use has caused himself and his friends to give up on him and wishing that they was the person that they use to be before their drug addiction. Given the way the song is written, I think that the second interpretation fits better.

For me, the song--and the movie, which is one of my all time favourites--is about someone who has found themselves in the liminal spaces of existence. About the erosion of what people consider "normal" mental states, the occasional suicidal ideation, and the attendant isolation of someone who has long been willing (or forced) to forego social norms in order to confront those things which most people refuse to face, even if it results in the spectacle of a life few would consider acceptable. About trying to put up a good front while resisting the siren songs that the people around you can't hear and won't comprehend, as they grant you abundant concern without a modicum of understanding. About trying to deal with the inevitable ontological distance that comes with seeing the world in a radically different way. If I sound opaque it's only because it's difficult to put these things into words on such short notice--this is, after all, a TH-cam comment. The movie is highly underrated; I cannot for the life of me understand why it's not immensely more popular.
