You don't use any of your information except for your own signature and your own initials. You are giving them carrier's info but since you have power to sign for them, you use your own signature and initials.

Yes, I can talk about that. I, myself, do not send invoices on behalf of carriers. Only for the trucks that run under my own MC#

You need to send an invoice to your carrier for the percentage of the gross amount your charging them.

I agree. I assume it's because each broker has their own policies and rules, that you have to agree with...

It would have to be a scanned image in PDF format. You can use Genius Scan or Cam Scam apps. They are free
