Sir this is my favourite utube channel of all time, im a medico, if i need to learn any topic and your video is there, i go for it without any doubt, i just wanna say pls never stop sharing your knowledge, idk how but you have the ability to simplify really complex topics, maybe u studied and revised many times....but they r just so helpful, along with that, you r such a fun loving and entertaining teacher, it adds cherry to the top, even your 2 hr video seem intrresting and never never boring.......you r really paying great contribution in making of some good doctors....i just bless you and hope u r happy healthy and cool as always. 🎉🎉🎉🎉🎉🎉🎉😊😊😊😊😊
Wow, this is so much helpful than all the "sophisticated" videos! A layman's request: a very early stage of glaucoma has been detected, I'm 65 years old male. From Canon OCT-HS100 v4.7.1: RNFL measurement NSTIN right avg 87, stdev 36.1 NSTIN left. avg 79, stdev 31.1 IOP was 23/25 (2019), 22/23(2021), 19/19 (2023). Next test one year from now. My questions would be: 1. What can I do with lifestyle (nutrients, I quit smoking years ago) 2. What is the right moment in term of progression of glaucoma to start thinking about corrective actions? Medicine or other? I hope you want to give your opinion, appreciate it!
This is the first time anti Glaucoma drugs made sense to me!! Thanks a ton to you wonderful human! Clear and fun way of teaching. I became a fan instantly ♥️♥️. Also punched the subscribe button 🤗
So u mean, common cause of glaucoma is open angle and close angle or narrow angle is less common. How about the mechanism of open angle glaucoma? You only explain about narrow angle glaucoma Sir

I need help, i was diagnosed with glaucoma 2 years ago, so I have been using Combigan, and today 01/02/2023 I went to a different hospital and I was diagnosed with keratoconus .. my question is can someone get both glaucoma and keratoconus?
Loved the explanation. I just had one question it says that BP and IOP are tied together with an increase in BP will lead to an increase in IOP, but wouldn't alpha 1 agonists also cause an increase in BP due to vasoconsitriction thus increasing IOP? So why do we use alpha 1?
Can someone clarify? I don't get this part 7:52 , If the pupil is constricted like he said wouldn't it become like 3:52 blocking the path = can't drain aqueous humor?
The prostaglandins act on uveoscleral outflow pathway- they increase the interstitial space within ciliary muscle. I don't think they widen canal of schlemn (trabecular outflow pathway) As per FA. 5:42

Should it be alpha 2 agonists? As alpha 1 cause mydriasis and should not be used for narrow-angle glaucoma.
Alpa 2 will cause the less production of the fluid and hence glaucoma will be reduced
Also I am confused won’t alpha 1 agonists cause increase in BP hence increase in blood flow and thus aqueous humour as well?
@@AbhiClair Bp increase due to vasoconstriction. Vasoconstriction leads to decreased AH production due to less blood flow to Cillary body.
@@AbhiClair They are used topically and thus systemic effects are not seen.

Ok so why are stimulant medications contra-indicated? Is it simply higher blood pressure = more aqueous humour production?
I really enjoyed how you explained the topic, it is concise, fun lol and easy to understand, especially with the precious crocodile tears! thank you

The etiology is high blood pressure so the medical student is right.
Not all the time, actually the real etiology is not known.

It depends on the route of administration (topical vs systemic) and the severity of the glaucoma (acute angle closure vs open angle).

Now,you knows the causes, what is the drug for cure. Name the drug.
It depends on the specific case and many other factors!
