      If you are really interested there’s a class named blood bank. It’s hard, yet it teaches these different variations of blood types , Rh itself has many different variations ,
      Example:
      DEc/cE
      DEC/ce
      Dce/ce ... etc
      There upon many different antibodies/ antigens you could go to an Antigram and see its different variations, theres ABO, Rh, Kell, Kidd, Diego, Duffy, M, N, S, Lewis, Lutheran and many more

    As a blood banker, thanks for this great video! One sticking point though: doctors don't do antibody screening or crossmatching, Medical Laboratory Scientists/Technologists do. The doctors rely on us to get safe blood for their patients.

    As an individual who has an almost non existent immune system. Plus I have a very rare type of anemia. I have to have blood transfusions often. Sometimes every two weeks.
    I thank everyone who donates their blood.

    Hi, an analyst in a bloodserology lab here. It's literally my job to do what you just described. I saw you missed a couple of spots. One being that the reason that AB0 and RhD have such a big impact is that they ALWAYS make antibodies, in fact the antibodies are present if everyone who doesn't have the antigen (because of gut microbes that have similar structures).
    Another thing that doctors can do is take blood from yourself if you have a rare blood type and then give it back once you need it (for instance with a planned surgery or pregnancy).
    Also, the screening you imply where you test for unexpected antibodies and crosstest with the donor blood? Unlike what you imply, that ALWAYS happens. It's an international prerequisite.

    Just want to point out: doctors do not, I repeat, do not screen for unexpected antibodies. They order that to be done, your friendly medical Lab scientist does the actual work.

    Important to understand "screening and cross-matching" is NOT done by doctors.
    It is done by Medical Lab Scientists in the lab.
    With a little knowledge, practically anyone can do the ABORh blood drop screening cards
    but the more complicated stuff is done in the lab by lab people who understand it.

    I once held 2 bags of blood by the same donor but taken 10 years apart. One had been frozen for that long and the second was taken because they wanted 2 bags available before the person with rare antibodies gave birth. I love when these episodes are about my work stuff.

      I read RH null is a blood disorder. My husband is 27 and said he just said he found out he is RH null.

      Monica Elaina Monica Elaina yup, unfortunately it is a disorder too, often called Rh deficiency syndrome. Rh antigens play a role in maintaining the integrity of the red blood cell, so Rh null people have more fragile RBC. Since these cells are more fragile, they break down more easily than normal RBCs, leading to anemia. Granted, I think most Rh null patients compensate for this anemia by producing more RBCs at once, but I’m not quite sure about that.

    I learned recently that I have selective IgA deficiency. So one type of antibody is missing from my body.
    Sometimes when people with selective IgA deficiency receive transfusions, they go into shock, because their body reacts badly to encountering IgA antibodies fof the first time.
    Good to know I guess.

      I think they simply need to prepare the PRBC with washed saline, and that will help elimnate the IgA in blood.
      Blood transfusions have many ways of prepartion to help specific people as enxample there is Washed PRBC as in your situation is helpful, some people need Filtered PRBC, or Irradiated PRBC ( depending on their immune status, organ transplant, etc.)

      MythoVirus is correct. Washed PRBC's are the product of choice for people who experience anaphylactic transfusion reactions most commonly due to IGA deficiency. However typically a person who is IGA deficient won't have a reaction the first time due to the body not having made the antibodies against IGA, it's typically the second exposure onward that is the problem.

    I absolutely loved this episode! As a biochemistry graduate I learned that there were many more antigens in the blood, but I didnt know why we mainly focused for Ab/Rh for transfusions. Thanks for the video! Could it be possible to make a video on the reason for the difference in the immune response for Ab/Rh groups compared to the others (explained from a molecular and evolutionary perspective)?

      My choir meets down the road from the donation centre off Oxford Street so it's normally less than four months between donations, I just book the next session while I'm eating my biscuits. That and last time I went there was nobody in for platelet donation so I got to use a comfy chair.
      Don't think that telling them is necessary; it must be on my file or else how would they have known to tell me?

      I’m immunosuppressed! I have Chron’s disease and with that comes a crappy immune system/meds that suppress my immune system. I needed a blood transfusion once during surgery, and it saved my life.

    My brother is AB- so fairly rare blood type. He donated blood and when he moved from the east coast to go to college in the west, they called him at home out there to give him the address of the nearest blood donation place.

      Ab- blood types can accept blood from any donee with - blood. He may be rare but is not exceptionally useful

      @@jeannie6011 Yeah, I have AB- as well. We're apparently 1% of the population. You are right to an extent. Our blood alone is not as useful as O- for example, but our plasma and platelets are. We are universal plasma and platelets donors. Which is also very useful.

    As a former Med Tech, I have stories for days about the joys of typing out dozens of units to find c e Jk(a) Jk(b) negative donor units whose surgeon didn't think about giving us a heads up days before surgery to have special blood on hand. Made me a better PA lol

    Cool video, slightly disappointed. My dudes, we perform type AND SCREENS on every individual getting blood. No one’s ever going to get blood on an ABO type alone (emergency situations aside). Also cross matches on every individual. Even in emergency situations, we keep segments of blood sent in emergencies so we can perform these tests retroactively when a blood draw is available. Also clinical laboratory scientists perform these tests, not physicians. Most doctors scratch their heads when the extended blood groups are in play.
    Maybe consult someone who works in immunohematology (hint: scientists) before thinking “ah I guess a doctor does this right?”

    I work in the frozen blood bank in the UK and work with some of the rarest blood types. A lot of rare blood is stored frozen in banks all over the world for up to 30 years, because if it was last as a normal donation it would only last for just over a month (depending on the country) and so might not be available when a patient needs it. I myself have sent and received blood from all over the world thanks to the international rare donor panel.

      It's a regulatory requirement by laboratory accreditation agencies and government agencies in the USA. You can however do electronic crossmatching which checks historical and current blood typing and antibody screen results against a donor unit in the computer system.

    When I was in highschool our class went to a biology exhibition in a university and they were demonstrating how to test your blood group, I tested my own blood group there although I'm afraid of needle but I was so dang happy cause I thought it was so cool 😂 .

    I'm a Specialist Biomedical Scientist, specialising in Transfusion. I have to say this video is orders of magnitude better than anything else I've seen online (EDIT: with the exception of BBguy)! I can't fault any of it. I especially love how you said Rh instead of Rhesus. I've seen an article about Rh null in a big newspaper before that thought Rh null blood is *literally* golden. Good work!

      @@mrwog82 They're a similar problem, but on different antigens. Rh null affects the Rh antigens, Bombay affects ABO antigens. The biochemistry of how ABO and Rh are synthesised is quite different.

    As a med lab tech just waiting to hear about Bombay blood type because we had to learn about it even though our chances encountering it are practically nill. Edit- doctors do not perform blood bank testing that’s my job.

    There's actually all of the negative RH blood types in my family :D My dad is O-, my mum is AB- and me and my eldest brother is A-, whereas my older brother is B-.
    So if someone we know needs a blood transfusion, our family has the blood types for it XD (Since Negatives can donate blood to both positive bloodtypes and negative blood types XD)
    (Then again, they actually only need my dad, since O- can donate to all RH positive and negative blood types)

    Hey SciShow, doctors don't do antibody screens or crossmatches! Medical laboratory technicians, scientists, and technologists do. We're a pretty quiet part of the healthcare team, but we are the ones doing a majority of the science that goes into ensuring your health.

      Yeah that! Almost ALL the testing that gets done gets done by us lab rats. That glucose test, the cholesterol, all but the most obscure ones. And even then, it's us laboratory scientists that do the actual test work, measuring and mixing all the cocktails, then the doctor reviews the results and make the actual call of the result.

      Annmarie Myers our platelets clot better than all other types when transfused. they want you for trauma vics and cancer patients. that, and there's ALWAYS a blood shortage

      I graduated Med Tech school in 1983. It was complicated then. Now, in 2019 there are so many known blood antigens the book (AABB Tech Manual) is 3 times as thick!

      I think all RH neg and RH null cannot receive your O pos blood.
      But all possitives can receive your blood.
      O + is the most common one, mine is also O+

      bnet sucks yes, but it depends on how you store it. I’m a clinical laboratory science student who just finished his blood banking rotation so a lot of this stuff is fresh in my mind atm. Most blood is stored in a refrigerator between 1-6 degrees Celsius. Depending on the anticoagulant used in the donation process, these can last anywhere between 21 to 42 days. However, blood products of rare blood types are often frozen in glycerol using a controlled rate freezer. If you freeze these cells at less than -18 Celsius, they last for a year, and at less than -65 Celsius, they last for 10 years. Those with Rh null are a different story though. Their RBCs tend to be more fragile since Rh antigens play a role in maintaining red cell integrity. This makes it hard to freeze them, even in a controlled rate freezer, since they are more easily damaged by ice crystals. Therefore, it is much harder to preserve Rh null cells through freezing.

    The worst part is that you find out you're adopted while you learn about blood types.. Happened to a friend of mine back in 6th grade. She looked like she could be a combination of her adoptive parents, but the mom being A and the dad being AB didn't match with her being O.

    I have a weird-ish blood type! I found out when I wanted to donate a kidney. A+, non-A1. (Meaning that they didn't test to find what specific variant of A it is, but they know it's not the most common one.) It only has about 20% of the the A markers that A1 has. That means that, for donating/receiving blood, it's A, but for organ donation some of my organs can go to O recipients.
    me: "Sooo it's like A-light?"
    Doc: "... Yes. Yes! That's exactly right. I'm going to use that explanation next time."

      Adaginy hi there! I’m a clinical lab science student who just finished his clinical rotation in the blood bank, so I think it’s awesome that you have a different variant of the A antigen! More than likely, you just have the A2 variant. I don’t have the exact stats, but I remember learning in class that around 80% of group A people are A1, 19.9% are A2, and 0.1% are some other, rarer variant of the A antigen!

    When I first donated blood, at a Medical Center where I worked as a student,they told me that I was O +/- (positive as a donor, negative as a recipient.) I have SOME of the Rh variants, but not all.

      Sometimes. There is this blood group called 'Duffy' which has an A and B form (FyA and FyB) and also has an unstable form Fy. The FyA and FyB are used by malaria to enter your red blood cells and reproduce, which makes you sick. So having the unstable Fy variant is an advantage, and significantly higher rates of Fy are seen in places like Mozambique compared to areas where malaria isn't prevalent.
      As for the ABO system, variation is likely just because it doesn't cause any harm. The 'ancestral' variant is A, but it mutated to form B, and a different mutation caused an enzyme to stop working which results in O. This doesn't really cause any problems (except for donating blood) so there wasn't really selective pressure for or against these blood groups*

    My exhusband is A- and I'm B+. We had 2 children a female A- and a son B-. When he was going to have an operation neither of us could be used for an emergency. That has concerned me for years especially since B- is not a common blood type.

      Antibodies require antigens to work, so yeah, I'd say there is. Wouldn't be very easy to evolve without an immune system.
