Tennis Elbow, why it hurts? Everything You Need To Know - Dr. Nabil Ebraheim

  • Dr. Ebraheim’s educational animated video describes the condition of tennis elbow-lateral epicondylitis.
The tennis elbow is also known as lateral epicondylitis. It is an overuse injury that causes inflammation, tendinosis, and lateral elbow pain at the origin of a small muscle called the extensor carpi radialis brevis (ECRB). The pain is located on the outer side of the elbow. The pain may interfere with sleep and activities or even carry groceries. When this muscle (ECRB) is overloaded by repetitive activities, it causes pain on the outer side of the elbow.
I am going to go into some details about this condition.
It is the most common cause of elbow pain. 50% of tennis players develop elbows due to technical reasons and risk factors such as incorrect grip size or poor swing technique. This condition also affects workers who do heavy lifting, repetitive gripping, or use heavy tools. The pathology of the tendon shows disorganized collagen and angiofibroblastic hyperplasia. The condition usually starts with microtears of the extensor carpi radialis brevis origin due to eccentric overload, which is precipitated and aggravated by repetitive wrist extension and forearm pronation. The patient with tennis elbow will complain of pain with gripping, decreased grip strength, pain with repetitive wrist extension, and will have tenderness around the lateral elbow. Here is the area of pain for the tennis elbow and radial tunnel syndrome. Radial tunnel syndrome should be in the differential diagnosis of tennis elbow, especially if the patient is not recovering as expected from tennis elbow treatment (suspect that the patient has radial tunnel syndrome). Radial tunnel syndrome causes pain about 3-4cm distal and anterior to the lateral epicondyle. Radial tunnel syndrome occurs in about 5% of the patients due to compression of the posterior intraosseous nerve. You also need to rule out cervical disc problems and tricep tendonitis. The tennis elbow diagnosis was based on the symptoms and physical examination. The provocative test, which will increase the pain at the lateral epicondyle, is resisted wrist extension with the elbow fully extended. You can get X-rays of the elbow, but the X-rays are basically normal, and rarely do you find some calcification.
    Treatment of tennis elbow:
    - Activity modification
    - Nonsteroidal anti-inflammatory medication
    -Physical therapy (exceedingly eccentric exercises)
    -Injection (frequently used)
    -Steroid injection
    -May use PRP
    (Ultrasound guidance may be helpful)
    -Bracing
There is a 95% success rate with nonoperative treatment to relieve the pain. It may take up to 6-12 months to improve the condition.
Surgery is a resort for releasing and debriding the extensor carpi radialis brevis muscle origin. Excessive debridement and release of the tendon can lead to injury of the lateral collateral ligament, which causes the posterolateral rotatory instability of the elbow.

For elbow tendinitis you need to find a Medical Massage Therapist in your area. One to two treatments is generally curative but could take up to six weeks of treatment. There is no need for injection or surgery and it will resolve with manual therapy only. PT will not be effective unless they are performing manual therapy on the area, exercises will only irritate it until the lesson is resolved and the adhesion removed from the ECRB/U. Successfully treated dozens of cases, never had to refer out.

Here’s my story with tennis elbow
    Got it from lifting (gym)
After about 7 weeks I had 1x cortisol shot . To help things along I rested for a weak got back to about 75% . Got back to gym wore a brace and was ok just couldn’t do certain things but I could still train . Stayed that way for about 4 months no worse no better . Then I hurt my bulging disk so I thought we’ll maybe I can finally rest my elbow and I can nip this in bud . After 4 weeks of complete rest didn’t really do anything. So I thought what’s going here I found the tendon the spot where the pain was and I rubbed cream in and really went deep made it angry it flamed up straight away I then iced it took Anti Inflammatory s 2 days then put heat on it 3 times a day after a week my tennis elbow completely went . It’s like i unlock something idk 🤷‍♂️ maybe I had scared tissue
    That needed work

I don’t know if you respond to these, but I’ve had the injections. I’ve been through physical therapy. I’ve gone to a chiropractor, I’ve just completed shockwave therapy and laser treatments, but I am still not getting pst this. The radial tunnel syndrome I haven’t heard of, but it matches with some of my pain. Have I exhausted my options? Do I need to consult a surgeon?

I got this issue playing badminton. Played after a gap of 5 years and started smashing hard straight away. It almost took a year for elbow muscle to heel. Now say I have got it in my left arm after I played cricket (batting), I used a lot of bottom hand with left arm to place the ball in gaps in field. Must have overused the muscle during this. I am right handed so thought it only happens to your stronger arm. .

Worst year and a half of my life!
Exercises and icing only dulled the pain, repetitive motion did it for me so please don’t got ham and take rests when your arms need it!
I pray I never give myself or get tennis elbow again!
Please take care of your body! Just because you are capable of doing it doesn’t mean you aren’t doing damage!
For me it was like a busted hydraulic.. when I went to lift most things especially the heavier items my arms or arm would fall and the pain was something else… not as bad as carpal tunnel but crippling indeed!
    Avoid if possible!
It will destroy physically and mentally!
