So in a desperate effort to avoid these complicated and difficult feelings the protagonist seeks the deceptive comfort of excessive drinking, smoking and one night stands. Unfortunately this behavior makes the protagonist feel even worse and she begins to see herself as deeply flawed and even inherently bad. The music video focuses on the self-destructive behavior theme of the song. Illustrating the tragedy that sometimes when we can't break out of our self-destructive habits we can end up paying for it with our lives despite the best efforts of a guardian angel.

I think it's wonderful that she had put so much thought and energy into making a music video into great art that is why she is so legendary in the field of music video! p

Okay so from what i understood the creeper is not a creeper he actually was Madonna lover in the video but he died and Madonna couldn't cope and deal with his passing so she started going out and sleeping with random guys. The part you see Madonna kissing the creeper is actually an "appearing" ghost like and at that moment she realizes she really loves and miss him so she overdoses on the pills and dies hence the part we see them together "flying" in the end. Hope this makes sence lol
That creeper is actually the angel of death. That's why when she passed away in the end she was next to him. She was strangled by a stranger she had slept with.
