咒文有分詳版及簡版兩個版本。在日本留傳的有大咒、中咒、小咒，大咒三個版本：當中的大咒即詳版，中咒即簡版，至於小咒，日本學者佐藤任認為其實可能是把《無能勝明王真言》錯當成為本真言，因而出現「小咒」這個版本，因為在其他地方的傳承均沒有「小咒」。
怛姪他。唵。鞞殺逝。鞞薩逝。鞞薩社。三沒揭帝莎訶。

藥師灌頂真言，俗稱藥師咒，是藥師佛的根本真言（陀罗尼），是佛教界普遍誦念的知名咒語。根據《藥師琉璃光如來本願功德經》中釋迦摩尼佛在答覆曼殊師利的問題，要求世尊宣說發願消除業障諸佛的名號和殊勝功德時，世尊答覆，說出藥師琉璃光如來的十二本願，並說藥師琉璃光如來進入禪定於禪定中說出其陀罗尼，有兩項作用：一是在生病、受傷時誦念，祈求藥師佛為病人加持，減除病人的痛苦，幫助早日療癒。用法一般是在食物或飲水誦念一百零八遍，給病人服用。二是與藥師佛相應，希望命終之後，能夠往生藥師佛的東方佛國琉璃世界，也可以為亡靈誦念。
「曼殊室利！若見男子、女人有病苦者，應當一心，為彼病人，常清淨澡漱，或食、或藥、或無蟲水、咒一百八遍，與彼服食，所有病苦悉皆消滅。若有所求，志心念誦，皆得如是無病延年；命終之後，生彼世界，得不退轉，乃至菩提。是故曼殊室利！若有男子、女人，於彼藥師琉璃光如來，至心殷重，恭敬供養者，常持此咒，勿令廢忘。」

往生四十九天，最好是佛號不中斷。即使是往生的人，他自己功夫成片，或者念到一心不亂，這四十九天念佛給他迴向，決定增高他的品位。如果這個人自己功夫不行，沒有能往生得了，他到六道去受生，這個功德可以幫助他不墮惡道；他要往生善道，在善道裡面增長福慧，好事情，這是真實利益。所以念的時候要誠心誠意。我們看到一個例子，在生的時候不念佛，沒有接觸佛教，死了之後，四十九天，由於助念得是非常殊勝，她感受到了，也跟著念，居然在滿期，四十九天是滿期，七個七圓滿的那一天，她真的往生了。這就是中陰身能夠跟著大眾，人雖然走了，我們一般講，他靈魂沒有離開，依舊跟道場大眾一同共修，他往生了。這個事情，我們一點懷疑都沒有。 (本文節錄自【臨終助念答問】2集)
