I don't follow any specific combo here, however I start most packs with something similar to this sequence:
Realm of Anguish (hotbar) - RMB - W+F - Space - RMB - F - S+LMB - RMB - Shift+F - Shift+RMB - F - RMB - W+F - Space - S+F - S+RMB - F - Shift+LMB
If you need more accuracy, use S+F early.
Gear, addons, crystals and lightstones at the start of the video.

Yes, but Sage is very squishy, so its much more risky. Imo it is worth to use elixirs, they are all available on Central Market right now, atleast on EU.

You can see it at 0:11, add 12 from elixirs and we have 877 acc in total. With awakening sage you can go as low as 844 to have 100% hitrate on most skills (4%+), but then you need to hit all mobs with S+F, so for double pulling I recommend to have ~880, its much more convenient.
