Thanks EJ - I also found a work-around that you can override the actions that appear in the highlights panel when editing the Lightning Page. This allows you to manually add the Action to the highlights panel in the lightning page instead of the classic page layout.
Can confirm that enabling dynamic actions for the highlight panel will allow you to display the ScreenFlow action button. You can do this in the lightning page builder.

I'm having the same issue but it's not on the case object. It's on a custom object. Anything that I've googled, doesn't work. Any other ideas?
