You don't actually want to base optimisation decision around time, you want to use conversion volume combined with cost instead. Some ads you can turn off or scale within hours, if you are getting hundreds or thousands of conversions a day for example. If you are getting no conversions and you've spent multiple times your target CPA then I'd make changes.

For some of my clients there is barely demand for service specific keywords, so I thought a good alternative solution is to search for keyword categories that reach a broad audience but are less competitive? For example, one of my German clients offers laser hair removal and we don't target specific keywords because of the low demand. So we thought about promoting the general service but choose our keywords based on different wording-categories.

You have mentioned, that it is better to create different ads with different landing pages for different services. So each ad group will be targeted to different keywords. But what if different audiences use same keyword when the search for our service? Ideally, mas you suggest, each audience (b2b, b2c) should have its own landing page. How to structure Ads , so that google will know which ad copy should be shown to which audience? Or anyway, I should create one generic laanding page, and there route different audiences to another pages, dedicated to diff. audiences accordingly ? Thanks in advance
I actually think for the most part people will use different language when they search (at least the serious people that are likely to buy) in the scenario you have given. People will put "business" in their search if they want a business version or something like that. So I would still separate out the two. Yes there may be some overlap but that's ok.

Yes, I'd have a separate campaign for each city most likely. Yes you can target similarly across those as they are different locations.
