I remember I did a game trying to start up a plague to destroy an empire, and it just wouldn’t start because everyone wanted to stay inside instead of doing something with anyone else
Try infecting more people at the start or wait until the population density goes up. You could infect an army and then make that country declare war on another country. Trust me it will spread. But not destroy an empire only weaken
