- KARCO is pleased to announce the global release of its new 3D Animated video “Simple Mistake, Fatal Consequences”, a collection of depiction several real incidents on board vessel, dealing with accidental consumption of poisonous substances from water bottles.
Tragic fatalities caused by simple mistakes seem to haunt the maritime industry even today.
As a small humble gesture, KARCO would like to dedicate this 3D Animated video, to all the seafarers who lost their lives in service of the maritime industry.
KARCO wants to attempt to raise awareness amongst the global seafaring community to be more safe, to learn from their training and work towards ensuring safety of yourself and those of your colleagues at all times. This is an important responsibility you have towards your colleagues and your family.
We hope, all the seafarers learn from this 17 minute video, which is being offered at no charge(FOC) to any individual or shipowner/manager or any other organisation who are passionate and committed to the safety of seafarers onboard the vessel.

I learned from this video that drinking from unknown water bottles is common on ships.
For Indians maybe. Anyone with half a brain would not.

Maybe it is common in other countries where lifestyles are different. In some places sharing water bottles is common. In places where it is not done we would never even consider such aa thing unless you know that person and it is done. When I was in the military in the 70s sharing water was very common due to the conditions we were in.

Ah yes a random bottle of opened water randomly sitting around, truly I, a very smart person who is cleared to work on a floating natural disaster waiting to happen should take a little sip!
There was a case on the QE2 many years ago, where a Senior Officer had put boiler cleaner in a drinks bottle. Another officer serving drinks at an informal evening in the mess, went to the cabin, brought that bottle and severed up the drinks. All but one man survived, sadly, because he did not want his stomach to be pumped. The story is well documented and worth a read.
Lesson: don't' ever decant chemicals into non authorised containers. And make sure the container is marked with the correct details.
Imagine one, drinking random water bottles, two, filling a water bottle with hazardous chemicals, not marking it, putting it inside a fridge.

think the fridge one is really stupid, there was a case with concentrated vinegar, where a woman nearly died. She went into a little on street shop, that sells ur daily carp, newspapers etc and they had water bottle next to vinegar essence for some reason and both bottles even had similar look and color scheme. Sadly the customer didnt check either and I know she got hurt badly, not sure 100% seems this happens every now and then. I kno they changed labels for the vinegar essence to look different.

Rather sadly (in the UK) a Sheffield Council employee working in the parks department wanted some weedkiller. He filled a bottle (from memory it was a dark bottle) of a popular drinks brand with paraquat. For some bizarre reason he went to his mums house, left it in the fridge, where a family member saw it, and thought it was for anyone.
As there were no markings anywhere on the bottle to say it was not the drinks brand in the bottle, the person took a swig, instantly realised, spat it out. Sadly they died after an excruciating death.
What makes this case even more shocking is that the UK has very strict rules on the Control of Substances Hazardous to Health (COSHH) and the above should never have happened. Even down to the individual not removing a wrapper or marking the bottle....or better still not putting it in a fridge! But if you have stupid people, you can make useless the rules put in place to protect us all.
There was also a case on the QE2 many, many years ago of someone filling up a drink bottle with boiler cleaner, and that again was a UK registered ship, with a UK crew.
An eye opening video on the fatality onboard due to 'Simple Mistake'. The cases mentioned in the video are real cases and depicts the chain of events leading to the respective incidents. Very informative and engaging content along with mentioned precautions to be taken and guidelines to be followed to avoid the same in your work area. This video should be shared with more and more seafarers so that the message could be passed on to each and everyone in the Maritime fraternity. Kudos to Team KARCO!

Most modern water bottles have anti tamper caps. If the seal is broken don't Flippin drink from it.

I mean, honestly, I enjoyed this far more than the other AI slop out there that people have been putting together.
Much better than having to stare at a bunch of randomly arranged stock photos and irrelevant/boring footage while a soulless, disembodied voice drones on and on with unnecessary and seemingly unending exposition every other five minutes.
I think what made this one somewhat better was the fact that the audio was much smoother and it seemed that at least some effort was put into the script - it was accurate, understandable, and not a long-winded word salad.
And the visuals were clearly designed and put together for this specific purpose, which is nice because, you know…I like when the scenarios being described match with what is actually being shown instead of a bunch of Shutterstock and Getty frames slapped together in a PowerPoint presentation and then adjusted so that each slide crawls by at the approximate speed of molasses.
Basically, it’s the blatant laziness I don’t like when it comes to most channels’ preferred use of AI, not AI in itself being used as a tool. As with other commenters though, I did kind of see this as a parody at first 😭
The jarring, stilted movement when each model was “coughing,” their facial expressions being a bit delayed or unnatural, plus that dramatic horror movie track that seemed to be the water bottles’ theme music kinda undercut the whole seriousness of the subject matter 😅
Whether I stuck around for the unintentional humor or for the points mentioned previously, either way you guys earned a view so does it really matter much? Lol keep being unique and hopefully other boring AI creators will follow suit 🤷🏻♀️✌🏻

I smell everything because when I was very young(4-5 maybe) I grabbed sealed carton of milk and started drinking. It was spoiled and all that sour curdle went into my mouth🤢. I gagged severely and since then I always smell before I eat/drink anything.

The irony? If the anti-rust solution, manifold sample, vinegar, paint thinner, bleach, and electrochemical cleaner was mixed together, the chemicals would cancel each other out.
The remaining solution would be neutralized by the vinegar, and would be safe to drink field expedient synthetic water.

Should be teaching the workers not to pick up any bottle to drink out of even if it looks like a water bottle they should be teaching that in first grade this is common sense. I mean you wouldn't pick up any water bottle on bourbon street and just start drinking would you hope you wouldn't
I guess you've never taken I guess you've never taken a class on safe handling of chemicals or other potentially hazardous materials. One of the most important lessons is do not put it into an inappropriate container.
