My Nephrologist who prescribed me an Arb explained that is works by dialating the blood vessels in the kidneys. After watching this video that states the excessive dialation of the afferent arteriole causes hyperfiltraion I don't see how the vasodialion with in the kidney from the arb's is good for my kidneys. This seems like a contradiction. My question would be, do arbs and ace's increase tgfb? I am no scientist, but am researching for my life, and ace's make me feel awful but are meant to be protecting me.
I am not by any means a medical expert. I am currently a PA student and we learned that the reason we give ARBs/ACEs for patients with Chronic Kidney Disease/Chronic Renal Failure is to manage hypertension. Hypertension can exacerbate Chronic Kidney Disease really bad. So yes, while it is bad to give an ARB because it is toxic to your kidney, the benefit is to manage hypertension. This is predicated and assumption that the reason you are on an ARB is because you have Hypertension and Chronic Kidney Disease
they are decreasing the hyperfiltration(both ARBs and ACEs inhibitors) wich prevents further kidney damage(the kidney doesnt like hyperfiltration) and like they said in the comment above it does quite well treat the arterial hypertension wich can further damage the kidney, lowers the aldosterone levels there are diffrent things. Summary if you want to spare your kidneys and prevent further progression of CKD you must take ARBs or ACEs.

ARBs are dilating efferent arteriol, not afferent, which is useful in kidney protection.
