Question cuz I'm not sure if the game actually clarifies this... Does this mean that Ikutsuki also knew about Death being sealed away inside Makoto from the start? And that him awakening to his persona is what started the return of the 12 shadows? Or did he not and he just grossly misunderstood how Death would return? Like, did sacrificing Mitsuru's father even have any effect on Ryoji's arrival or would he have still turned up anyway? I thought he might have known from the start, cuz it would make sense if he probed Aigis' memories of fighting Death to prevent her from revealing his plans. I also remember there being a suspicious early game interaction that made Ikutsuki sound as if he deliberately tampered w Makoto's dorm plans. But idk. This whole scene is just a bit weird to me.
Okay I'm trying to answer your question with my own interpretation of these events, since I don't think it's ever explicitly stated. 1. Ikutsuki knowing about Death inside Makoto. I think he didn't knew since the beginning, I think he knew when Thanatos killed the Magician Arcana. His surprise seemed genuine. 2. Death and it's return. So I do think he knew about the significance and correlation between Thanatos and the Shadows. 3. Sacrifice of Mitsurus father. Okay heres my crack theory about this whole ordeal. I'm not sure if the sacrifice of him actively brought Ryoji into the normal world. BUT I do think that his sacrifice had a role. There's something I noticed about Thanatos' design. It has 8 "coffins", or something akin to it, on a chain. And there were 8 planned sacrifices present in this scene. The father of Mitsuru and everyone of SEES (besides Koromaru obv). And I personally do believe that Mitsurus Dad was a somewhat improvised sacrifice and that his place was supposed to be filled by Aragaki. But since he passed away... Well he had to improvise. 3. Ikutsuki knowing from the start. I can imagine him knowing about Death being sealed away in someone. Maybe he'd tamper with everyones dorm plan that fit the bill of the information he has. Idk his surprise of seeing Thanatos just seemed so genuine so eh.
Oh and about Ryojis return. I think there's more to this whole sacrifice ordeal. Maybe this would've sped up Nyx' return, or that he wanted to "awaken" Thanatos/Ryoji on his own terms, to either use Thanatos/Ryoji or maybe to somehow become Nyx' Avatar himself? I don't think he grossly misunderstood, but wanted to turn everything into his own favor
Ikutsuki dont really know how to handle a Gun. Plus he was shocked for a Moment so his Aim actually should missed Mitsurus Dad. And not been that deadly.

There is absolutely no way she'd have ever been able to defeat SEES let alone the Protagonist. She's weak to Zio and is going up against several enemy Persona users at once. Only a couple Zios from Akihiko Ken or Makoto would've incapacitated her easily. This whole scene makes zero sense.
