If your issue isn't solved then try to do the steps again, make sure you don't miss anything, and make sure no matter what you remove ALL bluetooth devices, a lot of people don't do this and you need to. If the issue still persists after this you may need to erase the phone and set it up again, or have the phone serviced. This problem is usually a software issue but in rare instances it can be a hardware issue, if that's the case then you would need to have the phone repaired.
I have this same problem, none of that fixed it, resetting the network settings also no help. Hearing aid functions no help, going for a full reset to defualt settings and see if that works.
If the steps here didn't work then most likely the problem is actually the phone itself and not an issue with the settings or glitch. I would still encourage you to do a full wipe and start over. Remember, you can always set up the phone brand new if you like and not load your Apple ID just to see if the issue is fixed. If it is then it 100% was a glitch with your settings but if issue persists then it's the phone. If you do wipe the phone, the audio is fixed, then you load your Apple ID and the phone starts having the same issue then you've got a glitch in your settings that are saved. I wish you good luck!
www.apple.com/iphone-12/specs/ - this is specs for an iphone 12 for example, if you scroll down you can see the exact location of the speakers. Both should be doing the same thing as far as voice/ringtone. The 'Speaker' option is what allows you to choose something like a bluetooth device to play a call or other audio, so it wouldn't have anything to do with choosing the top or bottom speaker on the phone itself.

Can’t hear anything on phone calls after battery change. I think guy that put battery in screwed the pooch
Is the speaker that one hears phone calls with different from the one that gives the ringtone or the one that you select with the 'Speaker' option?
