西側の上越市街地は４車線で市街地を少し外れると２車線、東側の山越えの区間のごく一部が２車線未満といった感じで段階的に狭くなっていっていますね。
狭隘といっても、すれ違いが困難な部分はなさそうけど、主要地方道としてはやはり貧弱ですよね。
上越市と柏崎市の山間部を東西に結ぶ道路は、やはり国道253号現道が改良されているし、さらに上沼道(国道253号バイパス)の建設も進められているので、地元住民以外はこの県道を利用する選択肢はあまりないかと思います。
